The Licensed Mental Health Counselor is responsible for completing trauma informed integrated mental health assessments, person centered treatment planning, individual and group therapy, and case management to service recipients of Eckert Youth. Services could include but are not limited to adult and adolescent specific treatment services, residential day treatment, intensive outpatient treatment, low intensity treatment consisting of group counseling, family education, individual sessions and aftercare services. Additional services could include information and referral services, ongoing assessment, person-centered treatment planning, and case management.
The Licensed Mental Health Counselor evaluates and assesses psychosocial needs, provides information and referrals to clients/families regarding community resources/services and identifies discharge planning needs.
Responsibilities and Duties
- Formulate diagnosis and person-centered treatment plan for each client.
- Complete necessary Electronic Health Records procedures for all clients.
- Provide therapy to individuals with complex diagnosis and multiple problems, using high-level therapy skills and an elevated level of professional decision-making skills.
- Participate in supervision and consult appropriately regarding client's needs and services.
- Participate in all phases of the admissions process as needed.
- Document provision of all services provided, meeting all practice and licensing standards.
- Receive telephone calls, providing information and referrals to appropriate services.
- Complete initial intake interview and assessment for each new client. Use this information to document signs, symptoms, physical, mental, emotional, social, and spiritual implications, and treatment recommendations.
- Coordinate care with other providers and insurance companies to share observations and recommendations as well as re-evaluate patients to determine progress throughout care.
- Develop and execute treatment plans. Monitor patient adherence to treatment plan and determine if goals are being met.
- Provide educational individual or group therapies with adults and adolescents.
- Provide group therapy and mental health education based on objectives identified in the treatment plan. This may include individual and family therapy as necessary.
- Model and educate staff on cultural and socioeconomic characteristics faced by clients served.
- Document timely and accurately the patient assessments, evaluations, and therapy notes.
- Exercise professional duty to warn in cases of child and vulnerable adult protection.
- Obtain releases of information and acknowledge CFR 42 laws when sharing recommendations with client, family/guardian, and other care providers for feedback.
- Develop and nurture mutual relationship and confidence with patients.
- Evaluate patients’ conditions to determine the level of care and assistance they need.
- Work with patients to come up with effective and realizable treatment techniques that will yield good results.
- Enhance clients’ cognitive and behavioral changes that are in line with the overall treatment.
- Evaluate the efficiency of all treatments administered to patients and identify the most effective ones.
- Understand the individuality of patients and come up with specific treatments that meet their individual conditions.
- Direct clients to available community resources which are beneficial to them.
- Assist in periodically measuring the results of the programs and services against program goals. Assist in implement program adjustments as needed to enhance services.
- Participate with organization’s leadership and staff in efforts to achieve and maintain COA accreditation.
- Assist in Direct Care Staff duties if the need arises.
- On-call responsibilities associated with the position.
