LMSW- Lakeview Center for Health and Wellness

The LMSW is responsible to evaluate client needs, provide direct clinical services, and participate in interdisciplinary team meetings/staffing. Provide direct psychiatric social work services within the inpatient setting. Maintain appropriate documentation and prepare reports as required. May provide supervision to subordinate staff/ students.

  • Provide counseling for individuals, groups, or families.
  • Obtain social history on every client admitted, assessment, housing, financial and discharge planning needs.
  • Document daily progress in notes. Review Bill of Rights and Health Care Proxy with each client.
  • Provide input into treatment plans.
  • Provide services as an individual therapist, including assessment of treatment needs, treatment planning, and coordination of services.
  • Provide clinical services in accordance with managed care regulations when indicated. Obtain authorizations for services as necessary.
  • Provide clinical supervision to subordinate staff or students when indicated. Provide consultation/education related to specialty area for other staff as indicated. Work with Discharge Coordinator to ensure proper discharge plan for clients.
  • Maintain accurate and timely documentation and prepare periodic reports as required.
  • Establish and maintain effective relationships with Division staff, other service providers, and the community at large.
  • Coordinate clients’ plans of treatment with other service providers as needed. Refer for additional/specialized services when indicated.
  • Establish and maintain effective relationships with Division staff, other service providers, and the community at large.
  • Maintain current professional knowledge of therapeutic concepts and techniques. Attend seminars, workshops and conferences when appropriate.
  • Participate in inservice education and competency-based activities as indicated.

Ability to work under stressful situations with clients, families, and other human services providers.

Master’s Degree in social work with NYS license. At least 3 years experience working with mentally ill adults.
