Are you our next Licensed Practical Nurse?

As a Licensed Care Professional, you will work across Wilder’s Home Care Services in the community and Customized Living Services provided at two Saint Paul Public Housing sites (Ravoux and Hamline Hi-Rises). Together with Wilder’s Healthy Aging & Caregiving Services, you will join a collaborative team that supports the health and independence of adults to age and be cared for in the community, critical to Wilder’s mission of improving lives today and for generations to come!

In your role, you will provide person-centered care to older adults and adults with chronic conditions. Under the direction of an RN, you will assist providers and others to manage healthcare needs, assess clients, and monitor medication needs of clients. Communicating regularly with an interdisciplinary team and completing documentation as directed is also essential.

Wilder takes the health and safety of our staff seriously; we are following all MDH guidelines, we have infection control protocols in place and we are providing personal protective equipment and other supplies to mitigate the risk of COVID-19. Vaccination or an approved exemption is required for this position by federal mandate.

Job Qualifications

LPN Licensure for the State of MN. One to three years. Home health nursing and geriatric nursing experience preferred. Knowledge of Medicare home care rules and regulations preferred.
