Cornerstone Care, Inc. is seeking a Full-Time Integrated Care Coordinator for our Rogersville Office location.

The candidate must possess

  • Bachelor's Degree in Psychology or Social Work
  • Other qualified candidates include a RN or LPN
  • Experienced Medical Assistants with knowledge of behavioral health will also be considered

Other Requirements:

  • Travel possible between Cornerstone Care locations
  • MUST possess knowledge or experience in behavioral health as well as basic medical knowledge
  • Previous outpatient clinic work preferred but not required.
  • Current Act 33, Act 34, and Act 114 clearances

Must provide proof of COVID vaccination

  • DUTIES AND RESPONSIBILITIES:

A. Patient Care

  • Assists in the detection of “at risk” patients whom could potentially require further Mental Health and Substance Abuse screening during daily primary care team huddles
  • Performs advanced screening of primary care patients for various Mental Health and Substance Abuse Disorders and is available for same-day and scheduled initial consultations with referred primary care patients
  • Provides a range of services including screening for common conditions, assessments and interventions related to chronic disease management.
  • Performs brief behavioral and cognitive interventions and conducts motivational interviewing to develop behavioral strategies aimed at symptom reduction
  • Provides psychoeducation and medical education materials for patients and their support systems
  • Triages patients with severe or high-risk behavioral health or substance abuse issues and refers such patients to mental health and substance abuse specialists as appropriate
  • Facilitates and oversees referrals to outpatient and inpatient treatment programs and supports a smooth transition into treatment
  • Performs brief, limited follow-up visits for selected patients and contacts patients post-hospitalization when appropriate
  • Assists in preventing relapse or morbidity in conditions that tend to recur over time
  • Assists patients with medication adherence and disease self-management techniques
  • Provides consultation to Primary Care Providers both onsite and by phone to enhance understanding of the patient and provides decision support for treatment planning and symptom monitoring
  • Maintains a visible presence in the Community Health Center and participates in clinical staff meetings and Quality Improvement activities as required.

B. Supervisory/Training:

  • Educates Primary Care Team in recognizing and treating Mental Health and Substance Abuse disorders using basic principles of brief behavioral and cognitive interventions
  • Assists in training the Medical Assistants and Primary Care Providers to feel more comfortable handling crisis situations and possible suicidality
  • Assists in training the Medical Assistants on making referrals for treatment when the Clinical Specialist is not available.
  • Assists in training the Medical Assistants and Primary Care Providers in Mental Health laws and regulations


C.
Administrative

  • Reports quality improvement measures required for Grants, UDS and Meaningful Use
  • Provides assistance in capturing program evaluation and fidelity measures
  • Participates in the Quality Management Program and Policies and Procedures of the Psychiatry and Counseling Department, maintains performance improvement standards, policies, and procedures that ensure safe and therapeutically effective and preventative mental health care and continuity of that care for patients and their families.

KNOWLEDGE AND SKILLS:

  • Makes quick and accurate clinical assessments of mental and behavioral conditions
  • Works with Primary Care Providers using a consultative style
  • Demonstrates knowledge of the principals and practices of behavioral health consultation, including assessment, brief intervention, and motivational interviewing skills, and their application to population based care for preventative and chronic care issues
  • Demonstrates strong working knowledge of behavioral medicine and evidence-based short-term therapy strategies for Mental Health and Substance Abuse populations
  • Works through brief patient contacts, including telephone consults
  • Demonstrates comprehensive understanding of the interrelationship between medical and psychosocial systems and their impact on overall health
  • Documents clinical activities in a comprehensive, clear, concise fashion on a same day basis
  • Organizes time so that unscheduled, same day services are routinely available to care team members
  • Works effectively as a team, interfacing with patients, primary care providers, consulting psychiatrists and mental health specialists, as well as with administrative and support staff
  • Manages complex personal interactions and deescalate confrontational or crisis situations in a calm, consistent and equitable manner
  • Ensures patient confidentiality and strictly adhere to HIPAA confidentiality laws
  • Demonstrates ability to act with minimal supervision on personal initiative, using sound clinical judgement
  • Solves problems individually and in a group setting
  • Multitasks in a fast-paced environment
  • Demonstrates excellent written and verbal communication skills
  • Adheres to safety and security issues
  • Demonstrates patient/customer service skills
  • Works with county, public, and private partners to meet the needs of the community
