Position Summary:

Center for Family Services is seeking an engaging, energetic, and consumer service experienced individual for our Access Center. The Intake Associate provides exceptional service by way of incoming/outgoing phone calls, emails and internal inquiries. The Intake Associate is responsible for facilitating all aspects of the pre-admission process. This includes; verifying program and funding eligibility, entering referral, demographic and billing information into our EHR systems, and scheduling intake appointments.

Work Status: Full Time

Duties and Responsibilities:

  • Engage callers with questions to determine appropriate service types and referral eligibility.
  • Enter referral, demographic, and billing information into EHR systems.
  • Verify funding sources, including insurances, co-payments, co-insurance and/or deductibles.
  • Ensure proper, concise and complete EHR documentation and follow up.
  • Complete initial appointment scheduling in EHR and provide information regarding appointment, including telehealth processes.
  • Communicate and manage necessary documentation with referral sources for case coordination.
  • Maintain communication with referral sources and community partners regarding referral status and general program information.
  • Develop a strong knowledge base of agency programs and stay current on relevant changes.
  • Work with Access Screeners and Program Managers to promote ease of entry into services.
  • Collaborate with Access Screeners in making appropriate referrals and linkages.
  • Maintain communication and work collaboratively with Program Managers to ensure program admissions.

Requirements:

  • HS Diploma required, Bachelors preferred
  • Bilingual preferred but not required
  • Strong organizational and interpersonal skills
  • Exceptional work ethic and multitasking skills to manage in a fast-paced environment
  • 2-3 years of customer service experience; Human Services experience or education preferred
  • Proficient with Microsoft Office applications and Google Suite operations; EHR use preferred
