Responsibilities
The Admissions Specialist is responsible for the complete, accurate, and timely registration and initial coordination and scheduling of patients to the Organization's services while maintaining exceptional customer service.
Responsibilities:
- Responsible for the initial entry, verification, and maintenance of information regarding new patients in all applicable computer software programs.
- Acts as initial service ambassador to referral sources, physicians, patients, caregivers and other external customers providing the highest quality service.
- Responsible for the complete, accurate and timely registration of patients to home care and/or hospice, including but not limited to demographic information, payor information, service needs.
- Acts as a clinical intermediary for the field staff including: updates or changes in physician orders, takes laboratory results and communicates results appropriately, and appropriately enters ICD-9 codes.
- Works with location and corporate employees to ensure all initial patient information is correct for clinical services and monthly billing.
- Provides verbal and written information on home care and/or community services available.
- Assists in training new personnel as requested by management.
- All other duties as assigned.
Qualifications
- Current LPN license in good standing with the state serviced.
- Minimum of two (2+) years clinical intake experience required
- Experience in Home Care or Hospice preferred
- Must have excellent oral and written communication skills
- Excellent attention to detail
