Summary: The Information and Referral Concierge will assist with outreach by showcasing our Company Partner, AgeWell Atlanta on social media and, when needed, at meetings, events, and programs – both virtually and in person. The Information and Referral Concierge will be the point-of -contact for Aging services and resources for older adults and caregivers and also possess the ability to communicate effectively with our specified target population and the professionals who serve them.

Qualifications:

  • Bachelor’s degree required. Master’s degree preferred.
  • Area of study in Social Work, Gerontology, or related area strongly preferred.
  • Knowledge of community resources and services for older adults and family caregivers.
  • Sensitivity to the unique needs of the populations served (older adults and caregivers) and the ability to communicate effectively with members of those populations.
  • Minimum two (2) years’ experience in a human service delivery field, preferably with older adults and/or their family caregivers.
  • Strong verbal and written communication skills.
  • Strong interpersonal, team building and problem-solving skills.
  • Ability to work independently, with limited oversight.
  • Ability to work well as part of a team that includes partners and staff from multiple agencies.
  • Excellent organizational and multitasking skills.
  • Proficient technology skills, including social media and ability to learn new tools and systems.

Responsibilities:

  • Serve as a resource on aging-related needs for older adults, partner agency staff, and community partners.
  • Receive/respond to inquiries from older adults, caregivers, and professionals/partners. These inquiries will come via telephone, website, email, and in-person.
  • Perform initial screening to identify needs and barriers to accessing services and programs.
  • In collaboration with the inquirer/client, determine if the identified needs can best be addressed within the services and programs provided by AgeWell Atlanta.
  • Perform “Warm Hand-Off Referrals” with the consent of the client to one or more AgeWell Atlanta partner programs/services.
  • Make external referrals when appropriate. Provide contact information for “out-of-network” agencies and encourage the inquirer/client to initiate contact.
  • Collect and enter data used for producing outcome-based reports, including call volume, demographics, services referred/connected to, services provided, and all other key metrics that have been identified.
  • Using data collected from inquiries, help to identify potential gaps in AgeWell Atlanta programs and services, and report those to the AgeWell Atlanta Manager.
  • Assist with outreach, raising awareness, and building relationships with older adults and caregivers, as well as providers and partners (e.g. healthcare providers, aging services providers). Mechanisms for outreach may include but are not limited to social media, virtual events, and tabling at “live” events.
  • Other responsibilities as assigned by the AgeWell Atlanta Manager and collaborative partners on the AgeWell Atlanta Operations Team.

Supervisory Responsibility: No supervisory responsibility.

Physical Requirements: Ability to lift up to 20 pounds. Ability to stand up to two hours at a time. Ability to sit at desk, work on a computer, and speak on a telephone.

Mental Requirements: Ability to gather/analyze data, ability to focus for extensive period of time (up to 2 hours at a time), ability to interact and communicate clearly both verbally and in writing.

Job Type: Full-time

Pay: Up to $50,000.00 per year
