Virtua has introduced a new, exciting, and innovative care delivery model– “Hospital at Home.” The Hospital at Home program provides our patients inpatient hospital level care within the comfort of their homes.

Each patient is selected by a criterion that is set forth by a panel of Physicians, Nurses, and Administrators. Hospital at Home allows patients to fully recover in their own environment.

Nursing is performed virtually and in-person, with care of patients in the program leveraging a team nursing approach. You are required to complete at least two in-person visits with patients admitted to the program per day.

Mileage reimbursement and all necessary equipment will be provided. DME is determined prior to discharge and is sent to the patient's home. The documentation, assessment, and any IV med administration ordered are performed during in-person visits.

We are looking for experienced nurses with recent Medical/Surgical or critical care background.

RN - Job Summary:

The Hospital at Home focused Registered Nurse will bring the essential elements of the inpatient care delivery model into the patient’s defined living space through the use of leveraged technology and remote clinical care coordination. The Hospital at Home RN provides quality care to patients through the implementation of the nursing process applied through a team nursing model. The Hospital at Home RN will access and document the care of patients and the RN will work collaboratively with a multi-disciplinary healthcare team to deliver therapeutic interventions within the patient’s home while maintaining high quality and safety standards.

FT/PT & Day/Night shifts available.

12-hour shift – 7a-7p / 7p-7a (eligible for shift diff. of 10-15%).

Every third weekend and every other holiday required.

Located @ 301 Lippincott - Marlton for morning huddles.

Required Experience:

• Minimum of 2-3 years of acute care nursing experience.
• Experience working in an emergency department setting or home health preferred.
• Excellent assessment skills and clinical judgement.

Required Education:

• Graduate of an accredited nursing program.
• BSN preferred.

Training / Certification / Licensure:

-Current State of New Jersey nursing license to practice as a registered nurse. -Current CPR certification and BLS.

• Must have current proof of valid driver's license and auto insurance.

Care Coordinator - Job Summary:

The Hospital at Home focused Care Coordinator will assess and identify qualifying patients utilizing defined social and clinical eligibility criteria for the Hospital at Home program. The RN Care Coordinator will coordinate the patient onboarding process into the Hospital at Home program and coordinate admission activities while facilitating communication with the Hospital at Home care team and support staff to ensure seamless orchestration of patient logistics, patient education, and clinical care. Provides quality care to patients through the implementation of the nursing process applied through a team nursing model while maintaining high quality and safety standards.

FT/PT & Day shifts available.

Located @ 301 Lippincott / Virtua Hospitals.

Required Experience:

• Minimum of 2 years of acute care nursing experience required.
• Experience working in an emergency department setting preferred.
• Ability to function independently and understand complex care management.

Required Education:

• Graduate of an accredited nursing program.
• BSN preferred.

Training / Certification / Licensure:
