This position is the main point of coordination for the client’s care through the agency. As a PHCS RN Case Manager (RNCM), you may see several clients a day and must be able to provide quality, compassionate care within scheduling constraints while also accounting for deadlines and regulations.
RNCM responsibilities may include but are not limited to:
- Head-to-toe comprehensive assessments
- Visits for medication, condition monitoring, and clinical documentation
- Creating / updating the plan of care, coordination with other home care professionals. Evaluating the effectiveness of nursing and/or aide services to the patient
- Assistance with ADLs, wound care, trach care, etc.
- Teaching and training activities
- Case management: Open, re-certify and assess client cases

- Must have reliable transportation with ability to travel to the client's home(s).
- FLEXIBLE HOURS! RN Case Managers may adjust their schedules, as long as they adhere to the patient's needs and care plan.

Working Locations:
Care is provided to each patient within their own home. We provide services throughout the Central Ohio region. Most cases are in the Southwestern Columbus areas such as Groveport, Whitehall, Canal Winchester, Reynoldsburg, Grove City, Obetz, etc.
We will do our best to match your caseload to your location in order to provide manageability and ease of coordination.

Qualifications:
- Valid license to practice nursing in the state of Ohio without restrictions
- Valid Driver's License and Auto Insurance
- Candidates must pass a BCI and FBI background check
- CPR / First Aid (or willingness to obtain upon hire)
- COVID-19 vaccination or qualified exemption

Note about Pay Rates:
This position is currently Part-Time however, there is room to grow into a Full-Time hourly or salaried position with the right candidate. Growth opportunity and salary to be discussed during interview and commensurate with experience as well as performance.
