Education/Credentials of a Director of Patient Care:

  • Graduate of approved school of nursing.
  • Registered Nurse with a valid Michigan license.
  • BSN, preferred.
  • Hospice certification, preferred.
  • Valid driver license and vehicle available with current auto insurance.

Essential Job Responsibilities of the Director of Patient Care:

  • Collaborates with leadership to carry out the organization’s goals and strategy related to patient care.
  • Plans, leads, develops, coordinates, and implements policies, processes, and training initiatives to support the organization’s patient care needs.
  • Monitors state, federal and accreditation regulations and standards and assist in educating staff on changes in the regulations.
  • Coordinates development and maintenance of clinical objectives, standards of practice, policy and procedure manuals and job descriptions.
  • Participates as a member of the Medical and QAPI Committees.
  • Assists with orientation of new staff members.
  • Responsible for the training, orientation, and annual performance reviews of the Supervisory staff members for the patient care program and counsels as need arises.
  • Responsible for verifying and approving payroll for direct report staff.
  • Establishes, implements, and evaluates HOL’s care, treatment, and services to comply with federal, state, local and CHAP regulations.
  • Assists in the review of the policies and procedures, forms, and protocols.
  • Reviews current and ongoing quality assurance and quality improvement changes to assure regulatory compliance.
  • Reviews standards of care and related policies and procedures annually and makes recommendations for committee review.
  • Reviews clinical staff documentation and record of care provided and assures compliance according to agency standards.
  • Coordinates proposed annual patient care program budget for President and CEO review and Board approval.
  • Promotes financial stewardship and adjusts staffing model as needed to remain within budget expectations.
  • Participates in direct care hospice training sessions.
  • Ensures all functions of the patient care team meets the regulatory requirements of licensing agencies.
  • Oversees accreditation process including CHAP and CMS Licensing.
  • Coordinates patient and family concern process.
  • Coordinates and maintains clinical contracts.
  • Coordinates Medical Director schedules and coverage.
  • Manages all aspects of the patient care program, including Interdisciplinary Team functions in conjunction with Clinical Supervisors.
  • Meets regularly with patient care staff to monitor and evaluate clinical practice and plans of care.
  • Assesses the training needs of the patient care staff and works with the Staff Educator and the Director of Volunteers to develop pertinent in-services.
  • Serves as resource person for pain and symptom control to hospice team.
  • 24/7 availability for patient services.
  • Participates in community outreach/education and census growth initiatives and coordinates efforts with Community Outreach Coordinator, Clinical Leadership, and Direct Care Staff.
  • Responsible for learning and utilizing technology made available by the agency to increase efficiency, communication, and performance of duties.
  • Be knowledgeable of and follow policies and procedures set by Hospice of Lenawee.
  • Holds all patient information in strictest confidence and discloses information and data only to persons authorized by Hospice of Lenawee.
  • Promotes a culture of philanthropy with internal and external stakeholders.
  • Performs other duties and responsibilities as determined appropriate by the President & CEO.
  • Demonstrates willingness and flexibility to work different and/or extended shifts when needed.
  • Presents appropriate professional appearance in accordance with Hospice of Lenawee policies.
  • Participates in supervisor on-call rotation.
  • Hospice of Lenawee direct care staff may have exposure to infectious diseases. Must comply with standard precautions to prevent the spread of infectious disease.
  • Attends staff meetings and completes all mandatory training.
  • Complies with attendance policy and is physically at assigned location to fulfill job responsibilities and functions.

  • Demonstrates ability to mentor patient care staff as independent practitioners in the home care setting.
  • Possesses knowledge of community health organizations, practices, and resources.
  • Demonstrates ability to make sound clinical judgments and act upon them in a timely manner.
  • Demonstrates ability to plan, assess, implement, and evaluate nursing care for Hospice of Lenawee patients and families.
  • Demonstrates ability to supervise and direct care given by others.
  • Demonstrates leadership and management skills.
  • Demonstrates excellent customer service, both internally and externally.
  • Possesses strong communication skills and the ability to interact positively and professionally with patients, families, and other team members.
  • Demonstrates ability to function as a team member, collaborate with coworkers on different shifts, positions, and departments to promote teamwork and inclusion.
  • Demonstrates interest, compassion, and empathy for the terminally ill and is comfortable with issues of death and dying.
  • Is self-aware and asks for help when the need arises.
  • Demonstrates ability to read, write, and follow direction and instruction.
  • Has the physical ability necessary for extensive standing, lifting, and pushing.
  • Demonstrates ability to prioritize and manage time effectively.

Experience of a Director of Patient Care:

  • Two years nursing experience, required.
  • Prior experience in a health care environment, home care or hospice preferred.
  • Experience in a leadership and supervisory position, required.
  • Equivalent combination of education and experience may be substituted for specific requirements.
  • Experience with dying patients and their families, hospice experience preferred.
  • Experience in health care, required.

Position Status/Classification: Exempt, Regular, Salary, Patient Care, Management. Stipend given for Holiday, Weekly and Supervisory On-Call duties. Compensated at Hourly wage rate, when working as Primary Nurse
