SUMMARY:
Responsible for job skills training to individuals with disabilities. Including transportation, hands on instruction and supervision of job operations, quality control, work flow and material handling to meet production demands in assigned job sites. Participates in the development and implementation of Individual Service Plan goals/ Behavior Plans to maximize each assigned client’s ability to function independently.
SUPERVISORY RESPONSIBILITY: None
ESSENTIAL DUTIES/FUNCTIONS:
This position requires such typical work operations and responsibilities as, but not limited to:
- Implements the daily operations of the job site, including setting up, training and assigning work to clients/employees. Instructing the clients/employees in the requirements of quality control.
- Job coach is a hands on person and is responsible for seeing that the work guaranteed the employer is accomplished; regardless of client/employee’s ability or disability. Acts as a liaison between onsite employer and Kuhn management.
- Provides input for specific programs according to client needs. Implements program goals for each client including observation and record keeping (charts, etc.) of clients’ progress on a daily basis. Insures confidentiality of all appropriate client records.
- Participates in all team meetings requested by Program Coordinator.
- Provides task analysis, time studies for all clients assigned as requested.
- Records production data, including daily worksheets, attendance and time sheets, invoices, material logging, inventory control.
- Maintains a safe physical environment within the assigned work area, insuring supervision of all material handling, work tools and equipment.
- Actively communicates and cooperates with all staff regarding production demands, personnel and client related issues.
- Develops and implements meaningful, work-related activities with clients during periods of unpaid downtime.
- Responsibility for more than one job site when appropriate.
- Works with Administration and other staff as appropriate to review and revise program procedures in accordance with best practices in the field.
- Responsible to safely transport client to/ from the job site and/ or to/ from home as required.
OTHER DUTIES:
- Works with and assists others as required. Performs additional duties at the request of the management.
- Responsible for performing work duties in a safe manner, adhering to all established safety procedures and providing input into enhancing overall company safety.
- Attends job relevant seminars/conferences as appropriate; attends all mandatory training sessions and maintains required certifications.
REQUIREMENTS:
- Associatesdegree in Human Services related field or High School education plus a minimum of 1 to 2 years direct service work experience in a human service area. Experience with disabled persons preferred. Strong verbal and written communication skills.
- Obtain and/or maintain CPR Certification, First Aid Training and PMT Certification.
- Valid Connecticut driver license (with Public Passenger Permit endorsement).
- Available means of transportation to and from work.
- Automobile insurance in force per Kuhn’s specifications when using personal vehicle for work related purposes.
PHYSICAL REQUIREMENTS:
Be able to lift up to 40 lbs., on feet up to 7 hours. Drive for long periods of time.
TITLE: CEP Facilitator
DEPT: Community Experience Program (CEP)
REPORTS TO: Senior Program Manager
STATUS: Non-Exempt
SUMMARY:
Responsible for the development and implementation of behavioral programs for up to three participants and/ or as required by participants’ supervision guidelines in Community Experience Program. Facilitator is a hands-on, direct care position.
SUPERVISORY RESPONSIBILITY: None
ESSENTIAL DUTIES/FUNCTIONS:
Position requires such typical work operations and responsibilities as, but not limited to:
- Responsible for day to day implementation of client programs, activities and schedules as developed with the Coordinator of DSO Programs.
- Assists Coordinator and Program Specialist in maintaining compliance with DDS program requirements.
- Assures the quality of programming, record keeping, data collection, behavioral planning and implementation, to include: activity schedules, individual program plans, progress notes, data collection charts and other program requirements.
- Assist the Coordinator of DSO to maintain best practices in service delivery standards. .
- Assist the Coordinator of DSO to achieve the goals of this program according to contract and performance objective guidelines in the contract with the Department of Developmental Services.
- Provide documentation necessary for the Coordinator and/ or Program Specialist to present at participants’ meetings, i.e.: IP, URR, Behavior Management Planning, and other program related meetings.
- Responsible for transporting clients to and from community outings, as well as, to and from home as necessary. Also, responsible for van and/ or lift van mileage paperwork (recording mileage, gasoline, receipts, passengers, etc.).

- Responsible for performing work duties in a safe manner, adhering to all established safety procedures and providing input into enhancing overall company safety.

REQUIREMENTS:
- Associates Degree in Human Service preferred. High School diploma, plus a minimum of three years of direct vocational related work experience in a Human Service field will be considered.
- Must possess clear writing ability, and strong communication skills.
- Available means of transportation.
- Working knowledge of sign language and experience with behavior modified techniques preferred.
- Obtain and/or maintain PMT (Physical Psychological Management Training), CPR certification and First Aid Training.
- Valid Connecticut drivers license (with Public Passenger Permit endorsement).
- Automobile insurance in force per Kuhn’s specifications when using personal vehicle for work related purposes.
PHYSICAL REQUIREMENTS:
Able to physically transport and transfer program participants, perform PMT, implementation of behavior plans which require physical assistants and emergency restraints.
