LOCATION: Irvington, New Jersey- Hybrid (Office/Remote) Work Environment

CLASSIFICATION: Full-Time/Exempt- 37.5 hours/week

ANNUAL SALARY: $55K-$60K

PRIMARY FUNCTION: The Connecting NJ (CNJ) Case Manager Supervisor is responsible for program implementation, and supervision of the Connecting NJ Case Managers in Hudson, Union, Passaic, Bergen, and Morris counties for Connecting NJ, data collection for evaluation and reporting purposes.

ESSENTIAL DUTIES:
Responsible for implementing CNJ Case Manager Program in Bergen, Morris, Passaic, Hudson, and Union Counties.
Oversee Connecting NJ (CNJ) Case Managers providing support with case management, care coordination, and data collection.
Maintain direct oversight of CNJ Case Managers’ participation and coordination with the Department of Children Protection & Permanency (DCP&P) Plans of Safe Care meetings.
Complete training for CNJ Case Managers as required by the Department of Children and Families.
The (CNJ) Case Manager Supervisor will conduct regular 1:1 supervision to CNJ Case Managers.
The (CNJ) Case Manager Supervisor will support CNJ Case Managers with the development and implementation of care plans for their clients.
Monitor data collection by the case management team and submit monthly/quarterly progress reports addressing process and outcome indicators that will help determine strengths and areas needing improvement.
Work closely with Early Childhood Systems of Care programs to help case managers to connect to the most appropriate and available services.
The CNJ Case Manager Supervisor will utilize skills in communication, marketing, and public relations, as well as be able to perform professional oral and written tasks (e.g., public speaking, grant writing).
The CNJ Case Manager Supervisor will demonstrate leadership and a willingness to take initiative, working both independently and in a team.
Work closely with the CHW program to coordinate the flow of clients between the two programs.
Coordinate with the CNJ Supervisor to support the flow of clients within the CNJ HUB.

PHYSICAL DEMANDS: Work is performed in a variety of office, health care, and community settings; may involve bending, color identification, gripping with hands and fingers, hearing voice conversation, keyboarding, kneeling, lifting up to 35 pounds, pulling, pushing, reaching, sitting, standing, stooping and walking. Successful performance requires specific vision abilities that include close vision. The employee will attend meetings at a variety of places and will need to drive. Occasional stair climbing is required.

WORK REQUIREMENTS: There are no unusual work conditions. The noise level at work is moderate. The employee will experience outside weather conditions while in transit to and from meetings or during special events. Must have a car and a valid driver’s license.

PHYSICAL EXAMINATION: Offer of employment is contingent upon the final candidate’s successful completion of a physical examination. If a provider selected by the Consortium conducts the physical examination, there will be no expense to the employee. If the employee opts to choose their own physician to conduct this examination, the employee will incur the expense.

MINIMUM EDUCATIONAL QUALIFICATIONS:
A Master’s Degree in a social service field (i.e., social work, public health, psychology, etc.) is preferred.

MINIMUM EXPERIENCE/SKILLS:
3 years of experience supervising staff
5 years of case management experience in human services in the public sector
Experience interacting with human service providers and agency leaders and an understanding of the role of evaluation in social services.
High level of familiarity with county health and social services required
Excellent written and verbal communication skills and computer literacy are required.
Ability to communicate clearly and professionally with a range of community stakeholders.
