GENERAL DESCRIPTION

Qualified Professional is responsible for delivering community-based treatment to adults with severe and persistent mental illness and/or substance use challenges. This is accomplished by assessing, planning, coordinating, and maintaining care, which emphasizes rehabilitation, and community inclusion for program participants. The ideal candidate will be able to work in a fast-paced setting, actively engaged member of a clinical team delivering services in various community settings, pose the ability to coordinate and monitor care, able to complete all clinical administrative tasks for continuity of care, compliance according to local Management Care Organizations (MCO), and State entities.

DETAILED JOB DESCRIPTION

CLINICAL RESPONSIBILITIES

  • Assumes primary responsibility and facilitation for the person-centered planning process for participants, which includes developing, implementing, evaluating, and revising overall recovery goal(s)/plan(s) in conjunction with individual treatment team and as participants needs change
  • Completes/maintains all clinical administrative duties outlined by the state and local MCO, such as the Person-Centered/Crisis Plan, NCTOPPS, Treatment Authorization Request, and Clinical Progress Notes, which is maintained in participant’s permanent record
  • Utilizes evidence-based, best practices treatment interventions and concepts with participants and natural supports as outlined in Person-Centered Plan
  • Facilitation of weekly face-to-face contact with participants, with 75% of contacts in the community
  • Provides direct and indirect care to individuals, their family, and other natural supports to teach behavioral symptom management techniques to alleviate and manage symptoms not reduced by medication management
  • Assess ongoing the psychiatric, substance use, and behavioral status/history as it relates to specific life domains such as, physical health, education/employment, social engagement/development, activities of daily living, and family structure and relationships
  • Refer, link, and coordinating care to community agencies/services to meet unmet needs, providing advocacy, and acting as a liaison/consult with community agencies and natural supports for maintaining engagement and involvement in participant’s services
  • Provides psychoeducation to participants, natural supports with an emphasis on recovery, and de-stigmatizing participant’s illness
  • Implement services using modalities/approaches: MI, Trans-theoretical Model, Harm Reduction, CB Techniques, and other evidence-based modalities
  • Provides on-call crisis interventions as First Responder for evenings and weekends based on rotating schedule
  • Transitioning participants to other services i.e. ACTT, Outpatient, SAIOP, Residential etc.

ADMINISTRATIVE RESPONSIBILITIES

  • Attend and actively participate in weekly Team Meetings
  • Attend and actively participate in individual supervisions
  • Attend collaborations, treatment teams, and agency meetings
  • Completes/maintains all clinical administrative duties outlined by the state and local MCO, such as the Person-Centered/Crisis Plan, NCTOPPS, Treatment Authorization Request, and Clinical Progress Notes, which is maintained in participant’s permanent record
  • MUST be able to submit progress notes within 24 to 48 hours of date of service in participant’s permanent record
  • Submit clinical forms, paperwork, and report(s) according to deadlines
  • Submit all Timesheets, Mileage, and other assigned paperwork according to deadlines

TECHNICAL SKILLS

  • Basic computer skills for using Microsoft Word
  • Ability to use Electronic Health Record system
  • Writing skills that include sentence structure, grammar, and use of clinical terminology

WORK EXPERIENCE REQUIREMENTS

  • Basic knowledge about severe and persistent mental illness and substance use disorders
  • Works within your scope of practice according to credential, training, and skills
  • Understand the various levels of care based on the continuum of services
  • Flexible and willing to work in a “non-traditional” work environment
  • Employ treatment interventions and activities as outlined in the NC Division of Medical Assistance Clinical Coverage Policy
  • Implements the use of evidence-based interventions and concepts to individuals and natural supports in community settings

EDUCATION/CREDENTIAL REQUIREMENTS

Minimal Education:

  • Master’s Degree with 1 year post bachelor’s degree work experience with adults and or children with severe and persistent mental illness
  • Human Service Degree with 2-years post bachelor’s degree work experience with adults and or children with severe and persistent mental illness
  • Non-Human Service Degree with 4-years post bachelor’s degree work experience with adults and or children with severe and persistent and mental illness

Completes and maintains all required trainings set forth by NC Division of Medical Assistance Clinical Coverage Policy: 8A-6 which is specific to Community Support Team and Amara Wellness, Inc.

Job Type: Full-time

Pay: $40,000.00 - $43,000.00 per year
