Monarch NC, a provider of high-quality and trauma-informed care for people with intellectual and developmental disabilities, mental illness and substance use disorders, is seeking a dedicated Community Specialist to join our Southern Pines, NC team.
The Community Specialist will develop a positive relationship with the individual receiving services and their family in a work, home, school, or community setting while assisting them in achieving their personal dreams and goals as designated in the individual goal plans, as well as providing periodic relief to the caregiver.
This is a 7-day supervised living position and involves closely monitoring the person supported and assisting with daily living skills.
WORK SCHEDULE:
- WEEK 1: Tuesday through Thursday, 1 PM - 9 PM
- WEEK 2: Tuesday through Thursday, 1 PM - 9 PM; Saturday and Sunday, 1 PM - 9 PM
Responsibilities include:
- Provide support to help person maintain their daily schedule.
- Assistance with meal preparation and dietary restrictions.
- Assistance with engagement in community activities, volunteer work, recreation centers, local churches, YMCA, and other activities.
- Assistance with exploring new hobbies, activities and interests and integrating them into the person's schedule.
- Promoting a healthy lifestyle and increased quality of life for the individual.
- Transportation to appointments and activities mentioned above.
- Daily charting and documentation.
- Overnight Shift is expected to stay awake the entire shift and must check on person we support every 15 minutes and document accordingly.
- Perform all other duties as assigned by the supervisor.
To qualify, candidates must have the following:
- A High School Diploma or equivalent is required.
- At least 1 year of experience working with people with intellectual disabilities, developmental disabilities or substance abuse issues.
- A valid Drivers License and auto insurance is also required.
