Community Health Services, Inc. is seeking a Community Outreach Coordinator! The Community Outreach Worker will organize and implement health education programs designed to promote community health and educate the community regarding the services provided by the organization. Assist clients experiencing adverse events by providing immediate life needs or connecting them to services that can deliver. Provide health education and care coordination services for migrant farm workers in accordance with the requirements of the Connecticut River Valley Farmworker Health Program grant program.
The Connecticut River Valley Farmworker Health Program ("CRVFHP") is a federally funded program of the Massachusetts League of Community Health Centers, Inc. ("the League"). The goal of the CRVFHP is to improve the health status of migratory and seasonal agricultural worker (MSAW) populations and their dependents/ families (MSAWs) in the Connecticut River Valley by providing primary health care, dental care, mental health care, outreach, health education, and case management services to eligible MSAWs.

- Be able to work in a deadline-driven environment
- Be able to drive to various work-site locations on demand
Minimum educational qualifications required:
- Bachelor’s degree in social work, social services, health care, education, or a related field and one (1) year of experience providing social services to families OR five (5) years of experience providing social services to families.
- Written and oral proficiency in Spanish.
- Must pass a Proficiency Spanish Test.
- Certified Applications Counselor (CAC) or ability to obtain the CAC designation within 30 days.
- CPR Certified or obtain certification within 90 days.
- One (1) year of experience working with vulnerable populations.
- Valid Connecticut Driver’s license.
- Reliable, insured vehicle.
