SUMMARY
This is a full-time non-exempt position primarily responsible for the performing Community Hospital Liaison duties and providing behavioral health Casemanagement services to adults. This position may also be required to attend treatment team planning at outlying community hospitals as needed. Casemanagement Services are provided to adults living in community as well as the general and forensic population. Due to the nature of the job responsibilities, primary work hours will be Monday through Friday, 7:30am to 3:30pm with a 30-minute lunch (37.5 hours/week) unless otherwise agreed upon.

This position requires the employee to perform their daily job duties and responsibilities in a structured work environment.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Includes the following, (other duties may be assigned as deemed necessary by management):
Community Hospital Duties:
Responsible for coordinating a positive and cooperative working relationship with the County Liaisons, the Community Hospital Staff and all casemanagement staff. Specific liaison duties include initial completion of Environmental Matrix, WHODAS, other pertinent assessment tools to open clients to BSU, attendance at treatment team meetings, case monitoring, linkage, discharge referral coordination. Attendance at client hearings and completion of all required paperwork and necessary documentation in compliance with agency, county and state regulations. Attendance at regular Community Hospital meetings as needed. Regular attendance at weekly agency staffing meetings.
Casemanagement and Service Coordination:
Responsible for assessing the behavioral health and psycho-social needs of clients. Referral, service linkage, community support networking and development and case monitoring are essential job duties. Documentation of all service management contacts are maintained in client charts. Liaison responsibilities include working in conjunction with various inter-agency treatment programs; i.e., aftercare, CRR, day treatment, outpatient, etc. Coordination and liaison responsibilities with other human services providers. Participation and attendance at interdisciplinary team meetings, utilization review, treatment plan staffing. Completes all POMS registrations, dispositions and non-billables accurately and promptly. Responsible for achieving assigned productivity rate.
Crisis Intervention/Hospitalization Coordination:
Works as a Base Service Unit Team Member. Provides crisis services including telephone, walk-in and mobile crisis services on an as-needed, assigned basis. Coordination of 302 and 201 hospitalization includes completion of all required legal documents and emergency phone coordination with county, local community hospitals, emergency medical services and police.
Professional Responsibilities:
Obtain necessary training in accordance with assigned staff development requirements, carry a cell phone during work hours for communication. Able to be flexible, adjust work hours based upon program and consumer needs as assigned by immediate supervisor. Display a professional standard of effective teamwork in the provision of Administrative Management Services. Perform related duties as assigned.

Education and/or Experience:
Bachelor's degree in Social Service related field. Previous Casemanagement experience preferred.
