Eagle View Community Health System (EVCHS) is a Federally Qualified Health Clinic (FQHC) located in rural Illinois on the banks of the Mississippi River. Eagle View has been providing quality medical, dental, and behavioral health care to Henderson County since 1979. Qualifications:
- Education: High school diploma or GED required. Associates Degree (Bachelor’s Degree preferred), in any of the following (or similar) disciplines: social work, speech communications, pre-law, business, marketing, education, English, public administration, health information.
- Work Experience: Knowledge of health plan programs and eligibility verification preferred.
Job Summary:
Assists individuals in accessing community resources, providing social support, providing basic health education, transporting individuals to and from medical appointments, and advocating for individuals and community health.
Scope of Work Responsibilities:
- Establishes trusting relationships with patients and their families while providing general support and encouragement.
- Conducts comprehensive assessments of individuals, families, and groups, and provides ongoing follow-up with basic motivational interviewing skills.
- Conducts eligibility determination, enrollment, and follow-up assistance to help uninsured patients in gaining access to healthcare coverage.
- Encourages patient to attend all appointments and links to medical facilities as needed.

Site Work Location:
Each employee has a primary work site(s) to which he/she is assigned. EVCHS, however, has multiple sites. Therefore, any employee may be asked to work at another site if the need arises. Administrative employees and contracted service providers may work from their homes, as needed or agreed upon in their contract.
Providers may utilize Telehealth/Telemedicine from their homes during regular business hours so long as they are licensed in the state in which the patient resides. No employee shall be asked to perform work outside his/her job description/capabilities.

While performing the duties of this job, the employee is regularly required to sit, use hand to finger, handle or feel objects, reach with the hands and arms, talk and hear. The employee is required to stand, walk, climb, occasionally balance, stoop and kneel. The employee must lift and/or move 10 pounds or more. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Ability to operate a motor vehicle.
