The Position

This is limited duration recruitment. Persons appointed to limited-term positions will be designated as limited-duration employees whose appointment shall not exceed two (2) years except for the extension of grants or funding from outside sources.

Portland Fire & Rescue is looking for Community Health Workers to join the team!

About the position:
The Community Health Worker facilitates access to social services and agencies through a range of activities such as outreach, community education, informal counseling, social support and advocacy. As part of the High Utilizer Program within Community Connect you will work directly with Fire Stations or a liaison to help connect with case workers and support teams for members of our community who may be depending on 911 services when other services may be available. You will also work with Firefighters to help address community needs that need more support after a 911 call.

Major Duties and Responsibilities:
  • Work with Fire Stations or a liaison to connect with service providers to clients who are high utilizers of 911
  • Assess health status and social determinants of health;
  • Conduct outreach to identify and engage community members around health and wellness activities;
  • Attend and participate in team meetings;
  • Individual care coordination and/or case management;
  • Schedule appointment for individuals in other health/social services agencies;
  • Provide advocacy to address health disparities at the individual and community levels.

About the Team:
At Portland Fire and Rescue we keep all communities protected through a combination of prevention, community health programs, and all-hazard response to fire, medical, natural disaster, and other emergencies. PF&R has five divisions under the direction of the Fire Chief: Emergency Operations, Community Health, Prevention, Training & Medical Services, and Management Services. PF&R has a leading role in the safety and welfare of the community. The public has high expectations of us, and, therefore, we must have an even higher expectation of ourselves. The High Utilizer Program lives within the Community Health Division which also houses the Community Health Assess and Treat teams and Portland Street Response. In addition to the High Utilizer Program Community Connect manages Community Health Workers for CHAT and PSR as well as our vaccination programs and Meds on Wheels.

COVID-19 Vaccine Requirement
As of October 18, 2021, all City employees must be fully vaccinated for COVID-19. This is required for all positions at the City of Portland. Proof of being fully vaccinated is required by date of hire. You may request an exception, for health or religious reasons, from getting the COVID-19 vaccine at the time a position is offered.

Work Location
Currently, many positions with the City of Portland are being performed remotely due to COVID-19. In the future, current positions classified as “full-time remote” could: 1) remain full-time remote, 2) become a remote/onsite mix, 3) revert to full-time at a city work location. As the situation evolves, the exact work location of the position in this recruitment may change based upon leadership direction and approval.

To Qualify

Applicants must specifically address and demonstrate in their responses to the supplemental questions and their résumé how their education, training, work experience, and/or lived experience, meets each of the following minimum qualifications:


  • Ability to work effectively with other colleagues and disciplines in multi/intra disciplinary settings with other first responders, and health and community agencies.
  • Ability to observe and recognize reactivity by clients and to effectively respond and mitigate these issues when they emerge.
  • Knowledge of local population demographic and health disparities to assets and provide resources to address disparities.
  • Experience with information-gathering, and problem-solving to facilitate resolution of client disturbances.
  • Knowledge and experience with racial, gender, class analysis working with diverse groups, and a clear understanding of cultural differences, especially as they relate to race, sexual orientation, gender-equality, socio-economic status and their intersections.
  • Two years of community-based experience providing advocacy and support which has included significant public contact.
Applicants must also possess:
  • Completion of 90-hour Community Health Worker Certification Training
  • Must be fully vaccinated for COVID-19 per City/State mandate or have an approved exception by date of hire.

The Recruitment Process

Only candidates who meet the minimum qualifications will be placed on the equally ranked eligible list. You have 14 days from the notice of the examination results to let us know if you disagree with your results. Please read the City of Portland Administrative Rule 3.01 for complete information. Additional evaluation may be required prior to establishment of the eligible list and/or final selection.
