Community Health Worker

HCHC is a non-profit, federally qualified health center with a mission to create access to high-quality integrated health care and to promote well-being for individuals, families, and our communities. We provide medical, dental, behavioral health, eye care services, and community programs to everyone, regardless of ability to pay.

Compensation: competitive with a $1500 sign-on bonus available!

Location: Worthington Health Center, Huntington Health Center, Community Center

RESPONSIBILITIES

  • Community Health Workers are public health workers who apply their unique understanding of the experience, language, and/or culture of the populations they serve in order to carry out one or more of the following roles.
  • Providing culturally appropriate health education, information, and outreach in community-based settings, such as homes, schools, clinics, shelters, local businesses, and community centers.
  • Bridging/culturally mediating between individuals, communities, and health and human services, including actively building individual and community capacity.
  • Assuring that people access the services they need.
  • Providing direct services, such as informal counseling, social support, care coordination, and health screenings; and Advocating for individual and community needs.

QUALIFICATIONS

  • CHW Core Competency Training strongly suggested, but will consider supporting candidates who display an aptitude for this work in obtaining certification qualifications.
  • Culturally and linguistically qualified to work within population.
  • Demonstrated commitment to impacting at risk patients
  • Ability to work independently and as a team member across multicultural settings
  • Detail-oriented with the ability to multitask
  • Computer literate, experience with electronic medical record preferred
  • Strong time management, organizational and planning skills
  • Reliable transportation required and valid driver's license.
  • Knowledge of area culture and resources required.
