We are looking to add two more Community Health Navigators with good critical thinking skills and strong emotional intelligence to join our team.

The Wake County Medical Society Community Health Foundation (WCMSCHF) is a 501(c)3 that manages various programs that provide access to and improve health care for the indigent and medically underserved populations in Wake and surrounding Counties.

Project Access of Wake & Franklin Counties (PA), the flagship program under WCMSCHF, serves to provide connection to primary care as well as access to donated specialty, and diagnostic care while also improving health outcomes of enrollees.

POSITION SUMMARY: The Project Access (PA) Community Health Navigator (CHN) provides telephonic, or community-based engagement with identified PA enrollees at risk of non-engagement and/or non-compliance with the primary care provider. The CHN works to ensure a holistic, whole-person approach is taken towards affirming the patient/provider relationship and ensuring identified specialty and diagnostic care services are received.

JOB DUTIES

Patient Engagement

The CHN assists in establishing a Primary Care Home (PCH) as needed; makes referrals to resources addressing social determinants of health as needed; assists patients in preparing for and understanding the relevant details of their medical appointments; supports multidisciplinary health teams while engaging with the Project Access of Wake (PA) enrollees.

Provides social support by listening to the concerns of the patient and their family; appropriately communicates these concerns to the healthcare team; provides information about healthy behaviors and supports patients in reducing health-related risk behaviors in ways that patients understand and accept; provides coaching as needed to assist in understanding the provider/patient relationship for disease management, medication compliance, specialty services recommended by the primary care provider and/or any other general healthcare topics

Assists with problem-solving strategies to overcome barriers to care using a step-by-step process when teaching skills while providing positive reinforcement and encouragement

QUALIFICATIONS

· High School Diploma or GED required

· Community Health Worker Certification

  • Fluency in speaking, reading, and writing in Spanish with a knowledge of medical (illness-related) terminology is required.

· Minimum one year of experience in community outreach and resource coordination

· Have first-hand knowledge of the communities served and knowledge about community resources and services

· Fully understand social determinants of health and how they impact the community’s health

· Proficient in Microsoft 365

· Proficient in data entry/CM documentation in EMR system

· Must possess a valid driver’s license

· Reliable personal vehicle is required for travel between work sites
