Family Centers is seeking a Community Health Worker for its Screening Connecticut’s Youth Program (SCY). The selected candidate will provide outreach, screening, advocacy, and barrier intervention for youth and their families identified as at-risk for mental health indicators and will ensure the coordinated access to community-based supports and behavioral health treatment resources as needed.

KEY DUTIES & RESPONSIBILITIES:

• Conduct SCY program outreach to identify youth and their families
• Administer approved SCY screening tools to all consenting program participants
• Schedule mental health intakes for eligible, consenting participants
• Verify attendance at mental health intake
• Identify and mitigate barriers to participant attendance at mental health appointments
• Perform patient navigation and facilitate patient/client access to appropriate mental health providers and social services resources internally within the agency and externally in the region/state
• Work with other CHWs to share new community resources
• Attend all program and skill development training courses/webinars and meetings assigned or approved by Program Manager
• Document each client encounter
• Prepare reports and documents as needed or requested
• Attend regular team meetings
• Other duties as assigned

MINIMUM QUALIFICATIONS:

• Community Health Worker Certification from Connecticut-approved provider
• Long-time resident of Greater Stamford CT community with good knowledge of resources and programs
• Excellent oral and written communication skills
• Ability to exercise judgment in the application of professional services
• Comfortable with community outreach, cold calls, and potential home visits
• Proficiency with computer data entry
• Prior experience as an outreach worker, case manager or family support worker
• Must have access to a car, valid driver’s license, and up-to-date auto insurance

WORKWEEK: General business hours, potential evening and weekend work dependent on client need.
