Community Health Liaison Job at Christiana Care Health Systems

In partnership with the New Castle County Police Department, ChristianaCare is seeing a Community Health Liaison to support the Police Behavioral Health Unit and serve as a liaison among providers, hospitals, peer counselors, and people seeking treatment for and addressing their mental health needs.
It is paramount that this position have the ability to support and promote collaboration between public safety and public health agencies to ensure that persons in crisis and their friends and family have access to recovery support services. The ability to research and analyze a range of existing community-based efforts to address mental illness is also required. Some responsibilities include:
- Conduct interviews with all new participants to establish baseline data at intake.
- Work with medical and clinical professionals and Hero Help Addiction to identify/explain appropriate level of care.
- Present treatment options, inclusive of co-occurring disorders if necessary.
- Conduct weekly wellness checks to confirm compliance with treatment.
- Work closely with the licensed clinician and police officer within the New Castle County Police Department Behavioral Health Unit to track that the level of care remains consistent and fitting for everyone evaluated by the team.
- Establish and maintain partnerships with treatment providers, health services, and support groups to enable efficient and comprehensive service delivery for program participants.
Desired applicants will possess a minimum of a Bachelor's degree in a related field (BSW preferred) and have 5-10 years' experience working with treatment providers, law enforcement and social service providers.
