We're looking for someone to join our team as a Community Health Educator who:
Promotes individual and population health through educational initiatives and health behavior changes. Develops, implements and establishes programs and initiatives that support healthy food options, physical activity and community wellness. Establishes, leads and maintains community partnerships. Attends community events, as assigned. Coordinates multiple initiatives and projects at any given time.
Essential Functions:
- Establishes, expands, facilitates and sustains initiatives, programs and partnerships that engage stakeholders and community members in order to address community health priorities. Assists behavioral health, medical and dental clinic staff with patient outreach and educational support. Facilitates the implementation of community-focused initiatives to promote health and prevent disease.
- Utilizes community owned wellness initiatives to conduct and facilitate multidisciplinary or inter agency teams (like the SNAHC Patient Workgroup and Worksite Wellness Teams) and classes, as they relate to chronic disease wellness and prevention.
- Maintains and develops relationships between health centers and community organizations, agencies and other stakeholders. Develops plans to enhance and improve community relationships and partnerships. Collaborates with appropriate staff to plan, implement and evaluate community programs and events. Conducts outreach activities at community events.
- Ensures services are delivered in a culturally appropriate manner. Maintains accurate and timely service documentation. Completes and submits monthly activity report to supervisors.
- Abides by industry practices to maintain a safe working environment, like proper PPEs and ergonomic postures. Reports safety issues and concerns to supervisors or safety officers. Reviews safety and health plans, ensures proper understanding and asks questions regarding safe practices in work areas.
- Performs other job-related duties as assigned.
You'll be successful with the following qualifications:
Education:
- Associate's/Technical Degree in healthcare or equivalent combination of education/related experience: Required
- Bachelor's Degree: Preferred
Work Experience:
- Three years relevant experience: Preferred
- Two years electronic medical records experience: Preferred
