Job Summary

Works within the community providing outreach to patients with complex medical and social needs. Acts as a resource to assist patients in navigating the health care system.

Job Duties

• Assists in the outreach of patients identified as medically complex.
• Collaborates with the multidisciplinary team in the reinforcement of the care plan.
• Engages patients by way of home visit, hospital visit and accompanying patients to Providers appointments as needed.
• Facilitates adherence to treatment plan by early identification and resolution of gaps in care.
• Participates in multidisciplinary team conference in order to maximize the treatment plan.
• Provides reinforcement of the treatment plan under the direction of the Case Manager and Licensed Social Worker.
• Appointment scheduling and clerical duties as needed.
• Builds relationships within the community in order to identify resources that are available to the patients.

Position Details

• Work Schedule: M-F 8am to 4:30pm. One day per week the schedule will go to 6pm.
• Travel: Yes, this role will provide care in a participant's home. The role is community-based and travel will be required. In addition, travel regionally for ad hoc meetings.
• Required experience: 3 years of related experience
• Preferred experience: home health, clerical, medical terminology or CCHW certificate is preferred.
