GENERAL DESCRIPTION OF POSITION
Training about tobacco initiatives and the CBTD program will be provided upon hire.
This position coordinates the KDHE Community Based Tobacco Disparities (CBTD) program, which uses a community-led approach to address populations that are disproportionately affected by tobacco use, tobacco dependence, and secondhand smoke exposure in Bourbon County, KS. The coordinator will work with residents of Bourbon County to develop or join a community coalition, develop and update a comprehensive program plan, create community-based policy, systems, and environmental (PSE) strategies and activities, and ultimately, create change in Bourbon County within the target populations. The CBTD program focuses heavily on systematically reducing and addressing the social determinants of health as it relates to tobacco prevention, use, and cessation.
Requirements:
ESSENTIAL DUTIES
1. Provide overall coordination of the KDHE Community Based Tobacco Disparity (CBTD) program, including managing the requirements, reporting, evaluation, and budget.
2. Serve as the liaison and primary contact with external partners, patients, residents, and CHC/SEK care teams as it relates to the CBTD program
3. Demonstrate expert knowledge of and continually optimize the CBTD program, including program requirements, all related workflows and services both internal and external, and expected program outcomes, among others.
4. Create or join a local community coalition that assists in development, assessment, planning, and implementation of the CBTD program.
5. Build positive rapport with formal and informal community leaders and residents to inform project goals and strategies, which may include periodic travel within the county. Attend community events, as needed.
6. Advocate for change and adoption of program initiatives in a variety of organization, school district, city, or county settings, such as the adoption of new district-wide policies, county ordinances, among others.
7. Demonstrate expert knowledge of CHC/SEK, community, and external resources, and effectively communicate these with all interested parties.
8. Utilize population health data to develop and monitor program goals, improve health outcomes, and advance community health indicators, which includes a program dashboard.
9. Work to address health disparities for vulnerable populations by looking to innovative workflows and processes, or contributing to operational and community-wide changes.
10. Demonstrate knowledge of tobacco use, and related chronic disease conditions, and develop education or patient engagement materials to help support patient self-management goals.
11. Provide tobacco cessation services or educational seminars to patients or community members, as needed or assigned.
12. Performs other duties as assigned.
EDUCATION, QUALIFICATIONS, AND CREDENTIALS
1. Bachelor’s degree in a related field, or Registered Nurse, or equivalent experience.
2. Master’s degree in healthcare or related degree, preferred.
3. Experience with community building or community led initiatives, preferred.
4. Strong leadership and public communication skills.
KNOWLEDGE, SKILLS AND ABILITIES
1. Must possess and demonstrate a high degree of leadership, organizational ability, and communication skills.
2. Social and cultural sensitivity appropriate to ethnically and economically diverse patient-and employee-base
3. Demonstrate knowledge of the rationale of appropriate patient care.
4. Communicates through appropriate channels. Use proper chain of command for patient complaints.
5. Ability to handle emergency situations calmly and effectively.
6. Must be computer literate.
7. Must have a strong working knowledge of the organization’s EHR.
8. Must be able to maintain good inter-personal relationship with co-workers and other members of the health care team and the organization.
9. Provide customer service in accordance to the organization’s mission.
10. Be courteous and respectful when interacting with patients and family members.
11. Maintain patient confidentiality in accordance to organization’s policy and procedure and HIPAA requirements.
WORKING CONDITIONS
While performing the duties of this Job, the employee is regularly required to sit; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is frequently required to stand. The employee is occasionally required to walk. The employee must regularly lift and /or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, color vision, peripheral vision, depth perception and ability to adjust focus.
