Title: Coding Documentation Specialist

Position Shift: Monday - Friday Hybrid Schedule after 6 Months

Position Summary:
· Reviews patients' medical record prior to scheduled Health Maintenance Visit (HMV) to ensure appropriate Hierarchical Condition Category (HCC) capture and recapture reflects accurate acuity and disease burden of the patient population we are serving. Must be knowledgeable in HCCs and ICD-10 codes. Contributes to a positive working environment and performs other duties as assigned or directed to enhance the overall efforts of the organization.

Credentials / Education:
· CPC, CCS, RHIT, or RHIA coding certification

HCC coding experience STRONGLY PREFERRED

RHC Experience Preferred

· High School Diploma or equivalent
· 3 Years Coding Experience
