Summary: Provide residents with routine daily nursing care and services in accordance with the resident"s assessment and care plan and as directed by supervisors.

Qualifications: Education:
  • Must possess, as a minimum, a 10th grade education.
  • Graduate of certified nursing program in accordance with state regulations.
Licenses/Certification:
  • Valid license as Certified Nursing Assistant (CNA) in accordance with laws of the state.
Experience:
  • One year experience as a CNA in hospital, skilled nursing or rehabilitative setting preferred but not required.

Summary of Essential Functions:

  • Provide licensed nursing care including Activities of Daily Living to residents that is consistent with person centered plan of care.
  • Ensure all residents are treated with respect, dignity with consideration of their choices and rights including self-determination.
  • Appropriate compliance with infection control procedures.
  • Performs other tasks as assigned.
  • Ensure meet documentation and service requirements in accordance with facility policies as well as insurance and reimbursement providers.
  • Assist residents to transfer, mobilze, positioning or ambulation per plan of care using devices and safe mechanics as recommended.
  • Assists and documents Bowel and Bladder programming for residents.
  • Prepare residents for exams, tests, procedures, support in escort residents to appointments and leisure activities as needed.
  • Collects specimens as instructed.
  • Communicate with shift changes or handoff of care of residents.
  • Observe and report signs and symptoms of pressure areas or skin breakdown to prevent or manage pressure injuries.
  • Communicate with nursing staff and interdisciplinary team about resident"s change of condition/status, behaviors, grievances, and preferences.
  • Check vitals, intake and weigh residents as scheduled and report any changes.
  • Setup hydration and nutrition needs for residents" preferences and needs as part of plan of care.
  • Perform administrative duties such as charting, prepare reports, medical forms, and billing charge sheets in accordance with policies and procedures.
  • Reports occupational exposure to blood, body fluids, infectious materials, and chemicals to your supervisor.
  • Inventory of resident personal items on admission and assist with storage and assist resident as needed with setup of personal items.
  • Participates with assigned in-services and trainings as required.
  • Clean and disinfect equipment between multi-resident use and report any broken or needed items to supervisor timely.
  • Demonstrate safety and follow proper body mechanics during execution of duties.
Knowledge/Skills/Abilities:
  • Must be able to read, write, speak, and understand English language.
  • Must demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served in this setting.
  • Must be a supportive team member, contribute to and be an example of teamwork and team concept.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/ personnel, and the general public.
  • Must possess the ability and willingness to work harmoniously with other personnel.
  • Must have patience, tact, cheerful disposition, and enthusiasm as well as willingness to handle difficult residents.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must be able to relate information concerning a resident"s condition to appropriate staff.
  • Must not pose a direct threat to the health and safety of other individuals in the workplace.
  • Skilled in direct patient care and able to provide the highest qualithy care.
  • Ability to maintain confidentiality.
  • Must meet physical, sensory requirements per policy manual for the position.
