Overview:
The Clinical Services Coordinator II (Internal Referrals) will provide clinical and administrative support functions for CARD’s clinical providers and triage team. The incumbent will liaise with patients/families, providers; Care Centers, and other internal programs.
Responsibilities:
  • Serve as the primary contact for providing internal referral information to families and professionals.
  • Complete intake coordination services for families prior to their appointment. This includes but is not limited to brief screening assessments, coordinating the receipt of background forms and previous evaluations; and providing resources for families while they wait.
  • Serve as the primary contact for internal KKI departments. This includes assisting, consulting, and coordinating services with members of the Triage team, Care Centers, CARD clinicians, or other KKI clinicians as necessary.
  • Communicate and coordinate waitlist services between Care Centers, clinicians, and departments.
  • Assist families with completing and sending forms/correspondence that will be sent to Medical Records, pharmacies, schools, and physicians’ offices.
  • Screen, channel and prioritize clinically-related calls and correspondence according to clinic protocols. Refer patient and family communications as appropriate to other KKI professionals including Nursing, Social Work, and therapeutic disciplines.
  • Support the medical records needs of providers to include sending raw data and loose filing to Medical Records. Organize and maintain all submitted background and history forms, Individualized Education Program’s (IEP), and other patient documents for providers. Scan and manage the green charts for obsolete records.
  • Update and maintain weekly referral lists; remove patients from pending list and provide immediate triage for families without email addresses.
  • Provide input and assist providers/therapists, the director, and the supervisor with identifying on-going organization and process improvement opportunities for CARD’s systems.
  • Assist in all clinic activities, events and workshops to include but not limited to ROAR, Open House, Annual Autism Conference, and Shared Treatment and Autism Resources (STAR).
Qualifications:
QUALIFICATIONS:

  • Must be proficient with Microsoft Office suite, including Outlook, Word, Excel, Access, etc.

EDUCATION:

  • High School Diploma or GED required.
  • Bachelors’ degree in human services related field or healthcare administration desirable.

EXPERIENCE:

  • Three to four years of previous administrative support (or other relevant, higher level experience)
  • Experience working in a medical setting is preferred.
  • Previous experience working with children with autism spectrum disorders is preferred.
