We are seeking an experienced and customer minded Clinical Pharmacist to join our Trinity Health PACE team.

This position is a hybrid remote position. Most of the work will be done remotely. However, candidate must live within 25 miles of a PACE facility due to occasions where work must be done on site.

Candidate must be licensed in and reside in one of the following states: Alabama, Delaware, Florida, Indiana, Louisiana, Massachusetts, Michigan, New Jersey, New York, North Carolina or Pennsylvania.

Trinity Health PACE ministers exclusively to the elderly through a coordinated PACE care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community. Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center.

This is a clinical position with no dispensing duties. It will include clinical review of patient medication, analysis or drug utilization, report generation, analysis of drug tends and communication and presentations to leaders with regard to compliance, audits and/or other requested data.

What you will do:

  • Oversees and manages Part D Compliance Program, including the FWA Program, to ensure compliance with all applicable laws and to meet Medicare prescription drug plan requirements, including oversight of Part D annual workplans.

  • Coordinates with Finance as needed to ensure compliance with all requirements for financial and regulatory audits.

  • Coordinates auditing activities and develops corrective action plan as needed.

  • Attend and present at all quarterly Part D Compliance calls and PBM meetings.

  • Communicates and coordinates regularly with Chief Clinical and Compliance Officer, Regional Chief Medical Officers (CMO), Medical Directors, and Pharmacy Benefits Manager (PBM) to ensure Part D compliance and appropriate utilization of medications.

  • Collaborates with Medical Directors and PBM in reviewing prior authorizations and designated monthly reports. (B vs D, medical necessity, ESRD etc.)

  • Coordinates activities related to pharmacy and Part D that impact the PACE programs including working with internal departments and external sources as needed.

  • Collaborates with contracted pharmacy and PBM to manage pharmacoeconomic and participant safety metrics (PMPM cost/utilization, Acumen Reports, Milliman Reports etc.).

  • Oversees and monitors opioid utilization and oversight requirements.

  • Institutes and coordinates Medication Management Committee(s) in collaboration with the PBM to review pharmacoeconomic, pharmacotherapeutic, FWA and other pertinent trends. P

  • Provides consultation to PACE Organization nursing leadership to support consistent medication management practices across sites.

  • Coordinate with PACE Organization leadership to ensure vaccine procurement, administration and documentation.

  • Provides consultation to PACE Organization investigations related to all drug dispensing errors, medication, and pharmacy errors.

  • Partner with the education team to ensures colleagues are orientated about FWA during orientation and annually. Provide education as needed on various topics.

What you will need:

  • Must possess a comprehensive knowledge of Medicare Part D regulations, pharmacy operations, systems, and utilization management.

  • Degree in pharmacy from an ACPE (accredited school of pharmacy).

  • 3-5 years of experience overseeing pharmacy operations and regulatory compliance.

  • A combination of education and experience may be acceptable.

  • PharmD preferred.

  • Licensure as a pharmacist in at least one State supported by TH PACE required.

  • Demonstrates superior written and verbal communication and presentation skills appropriate for audience comprehension. Ability to communicate complex data in a clear and concise manner.

  • Must be action-oriented, manage conflict well, be customer focused, have high decision quality, flexibility to adapt to ongoing change and have organizational agility.

  • Possesses a high degree of personal accountability, responsibility and independent decision-making abilities with the skills to plan, organize, develop, implement and interpret programs, goals, objectives, policies and procedures of the organization in line with mission, vision, and philosophy of TH PACE.

  • Comprehensive to expert proficiency with Microsoft product suite (MS Word, Excel, Power Point, etc.); basic knowledge and experience with electronic mail and calendaring system.

  • Position requires frequent kneeling, crouching, reaching, lifting, sitting, walking, bending, and lifting.

  • Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.

  • May require travel to Livonia, MI (Corporate Office) or other PACE locations. Travel no more than 20%.
