About the Organization and Position

Well known Non-Profit Health Agency is hiring for a Clinical Care RN.

This wonderful non-profit serves people age 60 and older, adults living with disabilities, and their families and caregivers in Lynn, Lynnfield, Nahant, Saugus, and Swampscott, Massachusetts for more than 45 years. They provide a continuum of community health and social services to help people maintain their independence, safely and with dignity. Providing a broad range of services including home care services; nutrition programs; transportation assistance; housing supports; clinical and protective services etc. The agency recently received a second prestigious 3-year accreditation for excellence in case management and care coordination from the National Committee for Quality Assurance (NCQA).

The Clinical Care Nurse will complete program Comprehensive Assessments, Intakes and Care Plans. In addition, the Clinical Care Nurse will provide oversight to enrollees’ discharge plans from the hospital or ED as well as monitor ADT notifications for Care Transitions support.

Responsibilities

• Participate in all program evaluation and quality improvement activities related to the Community Partnership program
• Attend all required meetings as a supervisor of the program.
• Provide information about the option to enrollee
• Complete Comprehensive Assessments, Intakes and Medication reviews and Care Plans according to program guidelines.
• Ensure care plan uniquely represents enrollee, including a cohesive overview of the enrollee

strengths and challenges, interests and goals, and the accommodations provided to support enrolleedecision-making.

• Provide informed choice to enrollee when identifying different LTSS services and community

resource options.

• Monitor ADT notifications for Care Transition support.
• Provide Care Transition support to CPC and enrollee as needed.
• Monitor the enrollee's health, functional, and social status.

Experience

• Current MA RN License
• 3 Years of Clinical Experience
• 3 Years’ experience in community nursing
• Experience providing care management in a health and/or human services organization preferred.

Job Type: Full-time

Pay: $85,000.00 - $87,000.00 per year
