Quality Healthcare Development (QHD) is looking for a full-time Administrative Medical Assistant to provide support to the Quality Director and Nurse Care Coordinator.
The Administrative Medical Assistant will work directly with the Nurse Care Coordinator to assist patients, their families, and the Liberty Doctors health care team by identifying resources to improve quality of care while decreasing the cost. The Administrative Medical Assistant will collaborate and communicate with the Quality Director and Nurse Care Coordinator regarding all aspects of the Care Coordination Program.

Reports to: Quality Director / Chief Operating Officer
Location: North Charleston, SC
Essential Functions:
- Coordinates care across the continuum by working to close care gaps. Monitors hospital portals to identify patients who have recently been discharged. Supports the Nurse Care Coordinator in facilitating care post discharge.
- Create various reports to disseminate to primary care practices to help them understand their current performance in quality programs.
- Review vital signs for patients enrolled in the Remote Patient Monitoring Program and communicate findings to the patient’s primary care provider.
- Outreach to patients enrolled in the Remote Patient Monitoring Program to ensure engagement.
- Enter patient information into the Remote Patient Monitoring Clinical Dashboard and scheduling appropriate pathways.
- Complete special projects or assignments as needed to meet initiatives and/or objectives of the Quality department.
- Assists and coordinates QI projects as needed.
- Maintains all department files in an organized, accurate manner.
- Maintains and secures files of QI activities.
- Directs communication to the appropriate staff.
- Coordinates provider profiling activities.
- Additional duties as assigned.
Candidate Education:
- Required A High School Diploma or GED
- Required or equivalent work experience
- Medical Assistant Certification preferred
Candidate Experience:
- Required 2 years of experience in a medical office, HMO or hospital environment performing administrative duties with a clinical focus.
- Required Other Experience with medical terminology.
- Demonstrated written communication skills.
- Demonstrated interpersonal/verbal communication skills.
- Ability to communicate and make recommendations to upper management.
- Ability to multi-task
- Ability to work in a fast-paced environment with changing priorities.
- Ability to communicate effectively with people at all levels.
- Ability to arrange their own workday.
- Ability to strategize and integrate ideas for different practice settings.
Technical Skills:
- Microsoft Word, Excel, PowerPoint
- Electronic Medical Record Experience
Job Type: Full-time
Pay: $17.00 - $20.48 per hour
