Position Summary:
As an Unaccompanied Children (UC) Transitional Foster Care (TFC) Case Manager, this position is primarily responsible for providing casework and supportive services to children in foster care, foster families, and biological families referred to refugee and immigrant services TFC program.
This position is expected to function effectively with moderate supervision while following the guidelines given on procedures, along with agency, federal, and state regulatory requirements.

Essentials Duties and Responsibilities:
  • Completes trauma-focused initial intake assessment within 24 hours of arrival and ensures youth feel safe and are well oriented to the agency and foster home.
  • Promptly coordinates contact with youth sponsors immediately upon arrival, while also ensuring the sponsor obtains all applicable documentation such as the Family Reunification program (FRP) packet.
  • Identifies a sponsor by working with the consulate or equivalent resources to assign a sponsor for the youth, if unknown upon youth's arrival.
  • Effectively utilizes a trauma-focused lens to complete the FRP in a timely manner to ensure youth's safe release.
  • Ensures that all assigned youth receive mandatory services while in care to include but not limited to mental health, medical, recreational, educational, and other supportive services in accordance with the time expectations of the program.
  • Maintains accurate and current information on foster children in care, while ensuring Office of Refugee Reinstatement (ORR) and Catholic Charities West Michigan (CCWM's) internal documentation practices and reporting requirements are met.
  • Effectively leads the treatment team in conjunction with the youth's therapist.
  • Actively participates in weekly staff meetings with the treatment team and stakeholders.
  • Attends, facilitates, and assists with providing on-going education and support services to foster parents.
  • Attends weekly meetings with supervisor to consult on cases, to review case plan and direction, and to evaluate client participation and progress.
  • Co-leads education group sessions in conjunction with educational staff.
  • Assists the licensing team to ensure foster families are well trained and equipped to support the UC's in their home.
  • Schedules meetings with foster parents and foster children in the foster home on at least a monthly basis and address any concerns in service delivery.
  • Translates both written and verbal communication for program clientele and staff, if bi-lingual.
  • Attends trainings and required court hearings, if applicable.
  • Drives for Agency business.

  • Ability to build rapport with clients to help them adjust to new life circumstances.
  • Ability to provide excellent and compassionate customer service.
  • Ability to work independently and exercise good judgement, and a high level of confidentiality.
  • Ability to display excellent verbal and written communication skills.
  • Ability to demonstrated clinical, therapeutic, and crisis intervention skills.
  • Knowledge of the National Association of Social Work Code of Ethics and the NASW Standards for the practice of direct social work.
  • Knowledge of computer and typing skills sufficient to perform essential job functions.
  • Ability to relate to diverse populations and cultures while communicating with clients and the public in a courteous and effective manner.
  • Mature and stable judgment as well as sensitivity to various cultures and the unique history of refugees.
Recommended Employment Qualifications:
Education:
A bachelor's degree in behavioral sciences, human services or social services fields is required.

Experience:
  • A minimum of one year of experience in child and family services or counseling preferred.
  • Bilingual Spanish/English fluency strongly preferred.
  • A minimum of one year experience working with children and families from diverse cultures with sensitivity to cultural difference and norms strongly preferred.
Wage Range: $39,500-$46,305.00
