This position supports recovering and formerly homeless individuals living in Alcohol and Drug-Free Community (ADFC) housing by providing advocacy and recovery supports. The Case Manager acts to ensure the stability of individuals within the program properties and will be familiar with and able to access resources for clients, including alcohol and drug treatment, mental health treatment and any other identified needs.

The Successful Candidate:

This position supports clients being referred from the Department of Community Justice ARC unit and the case managers working in the programs at the Shoreline, Estate, Richard Harris, and the Henry. The PTP Case Manager will work closely with the Employment Access Center, Department of Community Corrections ARC programs, and other service providers; in order to coordinate services and ensure ongoing support of program participants, as they work on income enhancement, finding stable housing, and developing self-sufficiency. The case manager acts to ensure the stability of individuals within the programs, is familiar with and able to access resources for benefits/entitlements, alcohol and drug treatment, mental health treatment, and housing.

HOURS: Monday - Friday 8:00 a.m. - 4:30 p.m.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • 1. Provide case management services for special needs offenders referred by the Department of Community Justice.
    2. Stay in communication with clients, Department of Community Justice Transitional Service Unit staff, parole officers and treatment providers. Provide record keeping for community corrections, and ADFC.
    3. Complete intake/interview process of clients upon referral from Transitional Services Unit including: assessing needs, explaining building and ADFC policies and procedures.
    4. Assist clients, in collaboration with parole officers and all housing providers, to transition to housing that is more permanent.
    5. Update weekly goals and activities for each client and keep detailed progress notes. Create forms reflecting the current needs of the program.
    6. Work with parole officers to establish a safe discharge plan for clients who are non-compliant or have violated parole.
    7. Attend clinical staff meetings and case conferences in order to initiate aftercare plans for clients.
    8. Work as a team member with Central City Concern Portfolio Managers and all building staff.
    9. Develop and maintain working relationships with alcohol & drug corrections treatment providers, mental health providers and other outside resources for clients.
    10. Monitor client utilization of assigned unit.
    11. Be available for follow-up case management.
    12. Participate in compliance, quality assurance and quality improvement activities as directed.
    13. Transport clients to Social Security Offices, DHS Offices, Medical Appointments or other emergency transportation needs.
    14. Relationship/trust building with clients.
    15. Participate in training opportunities to advance skills and knowledge in mental health and addiction and criminality.
    16. Oversight of OYA and Imani beds if applicable. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
    17. Gather, edit, and refine program boilerplates and impact data. Help to keep files organized and updated.
    18. Adhere to all CCC policies, procedures, and guidelines.
    19. Attend all required meetings and trainings.
    20. Perform related duties as assigned, within the scope of the role.
    SKILLS AND ABILITIES:
    1. Demonstrate basic knowledge and skills in chemical dependency and mental health counseling techniques with individuals and groups.
    2. Knowledge of chemical dependency as a disease, treatment and intervention methods.
    3. Ability to respond to mental health crises in a clinically and culturally appropriate manner.
    4. Ability to work with ex-offenders in groups and as individuals.
    5. Ability to work as a team member and to establish priorities.
    6. Ability to display a non-judgmental attitude, and a willingness to learn new skills.
    7. Demonstrate ability to work without supervision.
    8. Demonstrate knowledge of community and social agencies, and self-help groups.
    9. Sufficient manual dexterity and physical ability to perform assigned tasks.
    10. Ability to communicate clearly and concisely both verbally and in writing.
    11. Knowledge of how deal with a combative tenant/client.
    12. Ability to work in an environment where people may be verbally hostile.
    13. Ability to manage time and meet deadlines.
    14. Ability to understand and follow verbal and written instructions.
    15. Ability to work courteously with the general public, tenants, and co-workers.
    16. Ability to maintain accurate records and necessary paperwork.
    17. Ability to learn and apply training instruction.
    18. Knowledge of de-escalation methods or ability to be trained in de-escalation methods.
    19. Ability to effectively communicate with clients, co-workers, corrections personnel, police, merchants, the public at large and supervisor.
    20. Ability to provide leadership.
    21. Ability to provide advocacy, as appropriate.
    22. Professional maintenance of appropriate boundaries.
    23. Ability to establish and maintain cooperative working relationships with those contacted during the course of work.
    MINIMUM QUALIFICATIONS:
    1. High School Diploma or GED and six months of experience working with the homeless population, behavioral health environment, human services, or related environment; OR 6 months experience in a high-volume customer service role.
    2. Physical ability to bend, stoop, kneel, squat, twist, reach, pull, lift heavy objects and climb stairs several times a day.
    3. Must not actively be on parole or probation.
    4. Ability to adhere to Central City Concern’s drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Act of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.
    5. Must pass a pre-employment drug screen and background check.
    6. May be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern.
    7. Must adhere to agency's non-discrimination policies.
    8. Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, lifestyles, and sexual orientations, and treat each individual with respect and dignity.

PREFERRED QUALIFICATIONS:

  • 1. Experience with ex-offenders.
    2. Valid driver's license preferred. If required to drive, must pass a DMV background check, and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy. If required to drive, must pass an initial driver's training within 60 days of being an approved driver and continued recertification training.
    3. Understanding of recovery terminology and ability to apply the concepts.
