SUPERVISOR:Program Manager

STATUS: Full-time/Non-exempt


PROGRAM SUMMARY:

The goal of HomeFirst’s Homelessness Prevention program is to prevent homelessness for at-risk households in the County of Santa Clara, California. We achieve this goal by providing time-limited rental subsidies, case management, mediation, and other support to program participants.

HomeFirst’s Homelessness Prevention program is a part of the Santa Clara County Homelessness Prevention System (HPS), a network of over 20 agencies working together to eliminate homelessness by preventing its occurrence.


DUTIES/RESPONSIBILITIES:

Direct Services (70-80%)

  • Maintain a caseload of 25 program participants, including both short and longer-term enrollments
  • Conduct pre-screens to determine the eligibility of prospective participants
  • Provide information and referral to community resources for those who do not meet eligibility criteria
  • Process intakes for those who meet eligibility criteria and are approved for admission
  • Conduct thorough participant needs assessments, including bio-psycho-social assessments, to support the development of individualized housing stability plans.
  • Provide direct and indirect assistance with financial management, crisis resolution, employment and benefits, advocacy, and other services as needed
  • Meet participants both in the office and in the field for community-based service delivery at a frequency determined by participants’ need
  • Work collaboratively with individual landlords, community partners, and other team members as appropriate to address participant housing stability.
  • Administer financial assistance as needed
  • Utilize supervision appropriately, maintain open lines of communication, and provide updates on service delivery
  • Learn and demonstrate knowledge of agency and program policies and procedures
  • Report any suspected cases of abuse and neglect in accordance with state-mandated reporter law
  • Engage in system-wide case conferencing and coordination efforts
  • Conduct outreach and community engagement activities

Documentation (10-20%)

  • Maintain accurate and timely records of services rendered, including but not limited to case notes, participant records, and financial transactions
  • Maintain all other program documentation following the policy
  • Participate in ongoing analysis of and program alignment with industry best practices
  • Proactively and consistently improve the quality and effectiveness of program services

General (10%)

  • Attend all meetings and required training relevant to this position
  • Assist with office-related tasks as needed
  • Assist with other duties assigned

QUALIFICATIONS:

Education and Experience

  • Required bachelor's degree and a minimum of 1 year of relevant work experience. Bachelor’s degree may be substituted by a high school diploma or GED and an additional two years of relevant work experience).

  • Bilingual, bicultural preferred
  • Desire to work with people of diverse socio-economic backgrounds

  • Ability to manage multiple projects concurrently
  • Strong written and oral communication skills
  • Demonstrated ability to prioritize and meet established deadlines
  • Exceptional organizational skills
  • Proficient with Microsoft Office
  • Ability to present oneself and the agency professionally to internal and external audiences
  • Ability to work as a member of a team
  • Sensitivity to the needs of people experiencing housing crises
  • Enjoy providing hands-on instruction in basic living and financial skills

  • Valid CA driver's license and auto insurance
  • Reliable personal vehicle
  • Ability to lift to 30 lbs. on occasion
  • Ability to visit participants in their homes, which may require walking upstairs
