Job Summary and Qualifications

As a Case Manager Social Worker, you will provide direct nursing care in accordance with established policies, procedures and protocols of the hospital. While providing administrative support on the unit, you will work also provide initial and ongoing psychosocial assessments for patients/families having complex psychosocial needs and/or medical complex diagnosis. You will also coordinate unit activities and assure accountability of all assigned tasks.

• Your Primary function in this role is to collaborate with the health care team and develops treatment goals and plans intervention that assist the patient and family to resolve psychosocial concerns that may arise as a result of illnesses, engagement in high-risk social behaviors (i.e. substance abuse), limited finances and/or physical disabilities.
• As a member of Case Management team, you will provide crisis and supportive counseling to enhance the patient’s and family’s problem solving and coping skills. Counsel's patient/families regarding advance medical directives.
• Document case management/social work assessments, treatment goals, interventions, outcomes, and discharge plans in the patient’s medical record congruent with departmental policies
• You will refer patients who require extended care post discharge to long-term care facilities. Once an appropriate bed is located, arrange for the patient’s transfer.
• You responsibility includes to Identify, track, and record Alternative Care Days (ACDs). Intervene to prevent or decrease ACDs as appropriate.
• Performs admission and concurrent review for all payers and maintains legible documentation as directed by the Utilization Management Plan.

REQUIREMENTS

• Required Two years previous employment as a Registered Nurse.
• Required bachelor's degree.
• Licensed a Registered Nurse in the Commonwealth of Virginia or has a Compact license.
• Basic Life Support (BLS) certification.
