Position Summary:
The Case Manager is responsible for establishing and maintaining a person-centered approach with all participants, identifying their personal interests and aspirations, and assuring supports (paid and natural) are in place to pursue them while focusing on community integration and independence.
Essential Job Functions:
- Provide a supportive environment that includes opportunities for individuals to explore, determine, and pursue personal preferences for vocational interests, development, and goals, social interests, and daily schedules to maximize independence and community integration.
- Maintain a high level of professionalism and function as an ambassador for Easterseals RI. Strategically identify person-centered placements and natural supports for participants through the utilization of networks and internal Business Developers.
- Establish, effectively partner, and foster relationships with all stakeholders including families, employers, funders, and targeted community groups and/or individuals to identify resources and natural supports for ESRI participants, assure satisfaction with services provided at all levels, and maximize interest in ESRI programs.
- Provide/assist with coordination of job development and follow-up retention services for participants working competitively.
- Transport participants to and from person centered activities, job development activities, job-related activities and other activities as required.
- Facilitate the development of and maintain Individual Service Plans (ISP's), Career Development Plans, and PATH's based on participant input, skills, interests, and needs.
- Coordinate service provisions with referring agencies including participation in regularly scheduled case conferences.
- Assist participants with researching and accessing supports including vocational training, financial, education, counseling, housing, medical services and other needed services.
- Participate in the hiring, training, supervision, and evaluations of Community Advocates working within the assigned team and facilitate staff meetings on a regular basis.
- Assure Community Advocates are familiar with the participants they are supporting as well as their interests, goals, and aspirations while maintaining a person-centered focus on community living and integration.
- Coordinate the weekly schedule of participants and assure appropriate support coverage is in place with Community Advocates and assure all documentation is timely, complete, and accurate.
- Maintain a schedule of frequent proactive visits to participants at their home, work, volunteer, community and recreational sites.
- Provide support and education to families, guardians, and other pertinent people within the participant's support network.
- Maintain paper and computerized participant records, complete required documentation and reporting including electronic ESRI database input with timeliness and accuracy. Record and appropriately utilize necessary data, daily progress notes, and complete all other ISP and program documentation to drive participant success including but not limited to daily progress notes and weekly, monthly, quarterly, bi-annual, and annual summaries.
- Ensure all participant information is maintained with the highest level of confidentiality and in accordance with HIPAA, state, and organizational regulations and policies.
- Ensure all participants are treated with dignity and respect in accordance with ESRI's Human Rights Policy and advocate for the choices, rights, and goals of each participant.
- Plan and coordinate on-site recruitment opportunities as appropriate; conduct tours of ESRI for potential participants as well as prospective employer partners.
- Provide orientation to new participants including review of participant handbook and placement expectations.
- Assist in proposal development and grant writing as needed.
- Execute all duties in accordance with organizational and funding source policies and procedures.
- Assist with the development and training of ESRI team members.
- Maintain knowledge of on-going program issues and developments as well as informing supervisor of program issues and concerns.
- Attends and actively participates in supervision and staff meetings.
- Provide direct service to participants, as necessary.

Education and Experience:
- Bachelor's Degree or equivalent in social work or related field, such as psychology, vocational rehabilitation, or occupational rehabilitation from an accredited institution with at least 1 year experience in a similar case management role (employment, training, workforce development, etc.) is preferred or six to ten years related experience and/or training, or equivalent combination of education and experience.
- Minimum of 2 years of successfully working in a community setting with individuals with I/DD or at-risk populations.
- Operational knowledge of services relating to the I/DD population, including person-centered planning, entitlement management, self-advocacy, ISP development and implementation, Career Development Planning, etc.
- A valid driver's license in state of residence.
- Qualifying criminal background and driving record.
- Must possess a safe, reliable, registered, insured, and inspected vehicle to transport participants as needed/required during work.
- Must be able to demonstrate competence in varied areas of training as identified by ESRI including but not limited to CPR, First Aid, behavior management, medication administration, and driving.
- Ability to intervene and model appropriate behavior in difficult or challenging situations.
- Capability to be patient and sensitive to individuals with varying disabilities and barriers.
- Must work within a multi-disciplinary team and as an individual contributor.
- Highly organized and detail-oriented with professional verbal, written, and interpersonal communications skills.
- Maintain a professional appearance and presentation.
- Capacity to operate and work with a computer including the proficient use of MS Word/Excel and the ability to operate various other software platforms.
- Ability and availability to work in 8-hour increments and/or as driven by operational needs.
