Case Management Coordinator Job at CVS Health
This is fully remote position. Candidate must reside in Dallas-Fort worth Area.
Utilizes critical thinking and judgment to collaborate and inform the case management process, in order to facilitate appropriate healthcare outcomes for members by providing care coordination, support and education for members through the use of care management tools and
resources.
Pay Range
The typical pay range for this role is:
Minimum: 19.50
Maximum: 38.99
Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.
Required Qualifications
Preferred Qualifications
Integrated Managed Care model experience preferred
Education
Bachelor's degree or non-licensed master level clinician required, with either degree being in behavioral health or human services preferred (psychology, social work, marriage and family therapy, counseling).

- 2 years experience in behavioral health, social services or human services field.
Preferred Qualifications

- Case management and discharge planning experience preferred
