About the Job

Responsibilities:

  • Demonstrate knowledge of community providers and supports.
  • Serve as Intensive Care Coordinator for McLean, Livingston, and DeWitt Counties.
  • Organize a spectrum of effective, community-based services and supports for children and youth with or at risk for mental health of other challenges and their families that is organized into a coordinated network, builds meaningful partnerships with families and youth, and addresses cultural and linguistic needs, in order to help them to function better at home, in school, in the community, and throughout life.
  • Cultivate and support provider and stakeholder relationships on identified cases with timely communication, inquiry, follow-up and integration of information into the care plan.
  • Complete needed psychosocial and mental health assessments to determine treatment recommendations.
  • Organize and facilitate Child and Family Team (CFT) meetings among providers involved with clients for a culture of respect, responsibility, and hope, utilizing a family-focused, systemic, strengths-based, and trauma-responsive model that are family and youth guided with the strengths and needs of the child and family determining the types and mix of services and supports provided
  • Assist the teams in developing and coordinating treatment and supportive services and in assessing treatment progress.
  • Deliver services in the least restrictive, most normative environments by providing services to the child and family's needs at their home or other community-based locations, as needed.
  • Participate in all required and requested training, coaching, and supervision processes established by HFS and supervisor.
  • Maintain records and reporting in accordance with HFS requirements and/or Medicaid billing requirements.
  • May need to be available for consultation with crisis providers with clients that are in a crisis event.
  • Meet for a minimum of one hour weekly, or as otherwise determined, with Intensive Care Coordination Supervisor, and attends group supervision meetings and staff meetings as directed. Prepare for supervision meetings as prescribed by supervisor.

Qualifications:

  • A Bachelor's degree, highly preferred in social work or human service degree as per current DCFS licensing standards (Required)
  • 1 year experience in mental health or human services position (Preferred)
  • Completion of DCFS criminal background check (Required)
  • Must be at least 21 years of age, obtain an insured vehicle, clean driving record, and three years driving record with a valid driver's license
