Founded in 1970, East Valley Community Health Center is a Federally Qualified Health Center (FQHC) who's services include providing personalized, affordable, high-quality medical, dental, vision and behavioral health care through a community-based network within the East San Gabriel Valley and Pomona Communities. Our staff practices patient-centered care by serving each patient with a personalized care plan that meets their individual needs. Our patients have access to support services that include, nutrition, health education, case management, pharmacy, lab, and x-ray at our health center locations. East Valley serves the health care needs of uninsured and underserved individuals and families throughout our 8 health center locations.

Purpose:
Oversee coordination of HIV positive clients with oral health and behavioral health services, implementation of policies and procedures, workflows, care, and appointment coordination. Oversee patient flow, data report for dedicated HIV positive clients for comprehensive retention in care.

  • *Responsibilities and functions:
  • Coordination to increase the linkage to dental and behavioral health services
  • Utilize the Well Health application as communication platform
  • Implement continuous quality improvement strategies
  • Provide care navigation services
  • Support expanded services and outreach
  • Policy, procedure, and workflow development
  • Implement a print and social media marketing strategy
  • Assist with IT enhancements
  • Restart the Patient Advisory Committee
  • Ability to deliver care in an ethical, non-judgemental, culturally competent, and non-discriminatory manner
  • Attend multidisciplinary case conferences, QI, and supervision meetings
  • Follow and understand HIPPA guidelines in relation to client care and confidentiality
  • Refer and link clients to other on-site services as needed and appropriate
  • Coordinate with other department staff to also increase retention in HIV medical care
  • Develop a collaborative relationship with various teams (medical, oral health, behavioral health, case management, etc.)
  • Document and complete all required documentation
  • Reduce gaps in services by promoting continuity of care
  • Participate in required trainings, conferences, and meetings
  • Perform other related duties as assigned

Requirements and qualifications:

  • A degree in social work or behavior health/human services related education
  • Bilingual (English and Spanish) required
  • Experience working with clients that are living with HIV/AIS and/or other vulnerable health populations, 3-5 years preferred
  • Experience working with underserved populations and appreciation for related challenges
  • Experience providing care navigation services for vulnerable health populations
  • Experience in communicating effectively and interacting with clients and staff with diverse social, cultural, and economic backgrounds
  • Experience using Electronic Health Records systems and interfaces preferred
  • Must have excellent written and verbal communication skills
