Highlighted Duties and Responsibilities:
- Provides group prenatal/childbirth education to patients participating in group prenatal healthcare
- Engages and works with prenatal mothers in group care setting;
- Maintains structure of prenatal groups by planning and executing the daily task required to coordinate a group;
- Co-facilitate with providers multiple prenatal groups on a weekly basis using the Centering
- Pregnancy model which incorporates three major components: assessment, education, and support;
- Connect with prenatal patients at 20-24 weeks of pregnancy to transition pregnant mothers from individual medical care into group medical care;
- Contacting group prenatal patients regarding missed group appointments to ensure that follow up is completed;
- Administers surveys to prenatal groups in an effort to understand the effectiveness of the Centering Pregnancy model and inputting that submitting that data to your supervisor for inputting purposes;
- Communicate with providers on the upcoming prenatal group sessions as well as email notices about changes to groups that may be taking place;
- Assists with orienting new prenatal mothers into the medical practice at CHRC as assigned by supervisor or providers;
- Work closely with the Care Coordinator for Prenatal Services and other Patient Services team members to provide adequate care to the prenatal patients;
- Documents all patient encounters, including face to face, telephone, and electronic communications, in the medical record in a timely fashion;
- Uses the patient alert system to prevent ‘missed opportunities’ for care;
- Manage prenatal group schedules with providers input;
- Collect and submit group related documents for submission into reporting system;
- Evaluate outcomes from surveys collected from prenatal groups.
Minimum Qualifications:
- High School Diploma/GED required, Bachelor’s degree in human services, public/community health, health education or social work, preferred;
- Experience providing health education in a group or individual setting;
- At least 2 years of experience in the health related field;
- Experience coordinating programs or initiatives and/or facilitating groups;
- Ability to express enthusiasm about breastfeeding and a desire to help mothers enjoy a positive experience;
- Knowledge of the Centering Pregnancy model (preferred) or experience working with a pregnancy model;
- Reliable transportation as traveling between two sites is required;
- Willingness to work evenings and weekends as needed;
- Commitment and ability to work with underserved populations.
