Our full-time non-exempt (hourly) Care Coordinator will help to support our mission and team so we can work in the most efficient and effective way. This position will be responsible for integrated patient care coordination under the direction of the CCBHC and Integrated Care Team Model. You will work Monday - Friday and have the opportunity for flexible scheduling and will work out of our Southport/Greenwood office location.

This role is part of the Certified Community Behavioral Health Centers (CCBHC) grant that A&C was awarded to expand and enhance behavioral health services in Marion and Johnson Counties. With this grant, A&C will implement more comprehensive mental and substance use treatment by adopting CCBHC service standards-an integrated and financially sustainable model for care delivery. Target populations for CCBHC services include individuals with serious mental health or addiction, including opioid disorders; children and adolescents with serious emotional disturbance; and individuals with co-occurring physical health disorders. CCBHCs provide person and family-centered services to any individual in need, regardless of their ability to pay or their place of residence.

What You'll Do

  • Participate in the development of the shared plan of care with other team members and safely coordinate implementations
  • Maintain and review patient records, charts and other pertinent information
  • Respond to and refer incoming and established patient treatment-related phone calls
  • Coordinate and follow patient referrals to specialty provider to enable and encourage patient attendance and follow up return
  • Coordinates discharge and transition planning for patient care
  • Ensures receival of patient medical records of any inpatient hospitalizations
  • Schedule any discharge follow up appointments in timely manner
  • Notify client’s team of any urgent crisis need that develop and ensures appropriate follow up care occurs
  • Assists in identifying and addressing barriers including, but not limited to, physical limitations, transportation issues, financial constraints, and lack of support to manage healthcare needs
  • Document and coordinate data, information, and clinical practices in multiple EMR’s and databases

Requirements:
  • Bachelor’s in Social Work or closely related field (required)
  • Valid driver's license and auto insurance
  • Knowledgeable of Medicaid/Medicare guidelines, Marketplace plans, and commercial insurance benefits and services, preferred
