Bereavement Coordinator Job at Bristol Hospice
JOB SUMMARY: The Bereavement Coordinator is responsible in planning, implementing and maintaining a bereavement program to meet the needs of families/caregivers from Bristol Hospice for up to one (1) year following the death of the individual hospice patient.
ESSENTIAL JOB FUNCTIONS:
- Coordinate the assessment and delivery of grief counseling needs and services (one-on-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers
- Plan for Bereavement Program development, expansion and refinement through annual evaluation of services
- Provide bereavement support to hospice personnel coping with work related grief through one-on-one counseling, supervision of interns providing one-on-one counseling, and referral to community resources
- Provide bereavement information and referral services to callers from the community
- Expand and maintain community resource list, as needed
- Assist as needed in evaluating and planning services to meet hospice personnel support needs (i.e., yearly retreats, weekly support groups)
- Function independently but seeks and accepts guidance from other members of the interdisciplinary group, or from members of the community
- Participate in hospice activities such as in-service education, hospice personnel meetings and relevant committees
- Advance professional knowledge and skills
- Actively participate in quality assessment performance improvement teams and activities
- Accept and perform other related duties and responsibilities, as required
REQUIRED EDUCATION & EXPERIENCE:
- Must possess a bachelors degree or equivalent theological degree from a college or university OR Master's degree in Social Work with qualifications of experienced clinical grief counseling
- Must have experience with families/caregivers and patients through death/dying issues
REQUIRED KNOWLEDGE, SKILLS & ABILITIES:
- Must possess the ability to work as a team member
- Must possess the knowledge and commitment to hospice philosophy
- Must demonstrate good verbal and written communication, and organization skills
- Must possess an understanding of grief response and experience with counseling individuals and/or in groups regarding loss
OTHER REQUIREMENTS:
- Must be able to travel often without notice
- Must be flexible in work hours
WORK ENVIRONMENT:
- Must comply with company policies, procedures and processes in addition to Hospice and Homecare rules and regulations
- Must comply with harassment and discrimination policies
- Must foster good working relationships
- Must be dependable and flexible in work schedule
- Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order
- Must be willing to submit to a criminal background check as well as a drug/alcohol screening upon hire and periodically thereafter
PHYSICAL ENVIRONMENT:
- May have to travel in inclement weather
- May be required to stand and sit for long periods
- Must be able to hear and see with or without aid
- Must be able to read and follow instructions as required
