About the role:
Job Title: Case Manager
Area of Focus: Behavioral Health
Program(s): BH, CCBHC
Experience & Credential Requirements: Qualifies as a Mental Health Professional (MHP) or will qualify within 18 months of hire; experience working in human services field and providing clinical services is preferred.
Immediate Supervisor: Community Resources Coordinator
Hours of Work: Full-time, 40 hours/week
Special Requirements: At least 21 years of age; schedules evening/weekend appointments as needed; must complete required registry clearances and criminal fingerprint background checks; after-hours crisis response rotation (for Bachelor’s level staff or higher); Travels to off-site/community based locations for direct service delivery; maintain CPR & First Aid Certification.
Skills & Knowledge: Demonstrates skills in the field of human services to adults or children, have demonstrated the ability to work with a team approach and accept supervision, and have demonstrated the ability to work constructively with clients, treatment resources and the community; Additional knowledge of case management processes, area resources/service systems, group facilitation, curriculum planning, teaching of basic living skills, written and oral communications, crisis intervention/conflict resolution; knowledge of peer recovery tools and methods are preferred.
Job duties include, but are not limited to:
- Serves as a member of behavioral health treatment team that is primarily responsible for assisting individuals and families in improving wellness and autonomy through advocacy, support, education, identification of service resources, and service facilitation
- Responsible for ensuring individuals have reduced barriers in accessing both internal and external health/behavioral health services by assisting with obtainment of resource needs
- Provides comprehensive consumer care and case management services including access to transportation, state and federal entitlements, medical care, psychiatric care, housing, and other basic needs to support stabilization and achieve/maintain recovery.
- Carries a client caseload as assigned.
- Development of recovery maintenance planning prior to discharge from services
- Will utilize evidence-based Illness Management & Recovery curriculum to facilitate group services for adults with severe and chronic mental illness
- Works collaboratively with the behavioral health treatment team to identify program goals and monitor progress in treatment; intervenes by way of conducting activities such as skill building, advocacy, recovery management planning, and support and consultation to improve the overall health and well-being of the client served and overall level of functioning in identified areas of life.
- Facilitates communication across agency staff and applicable programs.
- Assists as needed with setting up transitions in care and appropriate linkages for those entering or exiting higher levels of care.
- Teaches life-management and daily living skills to clients that translate to their natural environment with an emphasis on developing natural supports.
- Completes direct service billing requirements in an efficient and timely manner.
- Demonstrates active involvement in team/program meetings, supervision, and agency/community events and activities as assigned.
- Ability to screen, assess and provide interventions for clients that are exhibiting symptoms of trauma.
- Degreed MHP Only - Provides crisis response and intervention services to clients by utilizing short-term crisis intervention and conflict resolution techniques and referral to appropriate health and human service resources.
- Performs other duties as assigned.
Salary: $29,812 - $39,228 annually
