Essential Functions of the position are detailed below, and include any physical requirements below:
1.Works independently and consistently exercises discretion and sound judgment in conducting investigations and protective services to victims of abuse, neglect and exploitation to elderly individuals, people with disabilities (ages 18-59) and their families following the Illinois Department on Aging and Area Agency on Aging Adult Protective Services (APS) program standards and reporting procedures. Position rotates 24 hour on call for emergency clients;if on call, must maintain availability on a 24/7 basis by phone.
2. Complies fully with all APS regulations and program plan requirements which may include required time frames for priority level cases, and substantiated cases,and all service activity. Utilizes a variety of clinical modalities to engage and intervene with client,family members and others involved.
3. Prepares comprehensive initial intake, investigation reports, quarterly reassessment and closeouts on Adult Protective cases through manual/computerized assessment, case notes, and forms according to contract, funder and agency requirements.
4. Maintains up-to-date and accurate client files and databases, as required by agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory,licensing or accrediting bodies.
5. Provides thorough client updates during supervision as well as immediately notifying the supervisor of any risk of harm issues related to the client. Works with the Multi-Disciplinary Team Advisory Group and utilizes their input in case work.
6. Refers clients for required services, advocates for their needs, and ensures timely follow up regarding their receipt of services.
7.Completes all required and applicable training to maintain APS certification.
8. Presents Adult Protective Services education and awareness information to community agencies, organizations and groups. May chair/ lead community education and outreach activities.
9. May assist in orienting and training new staff.
10.May conduct intake services when needed.
PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned:
Lift up to 25 pounds.
Climb up and down up to 4 flights of stairs at a time.
Push and pull objects up to 50 pounds

Background check, including any program specific requirements.
Driver's License and reliable transportation.
Additional Requirements: May require travel to Springfield for three day training within first 6 months.

Education and Experience Requirements:
Preferred Relevant Education: Master's Degree
Minimum Relevant Education: Master's Degree.

Minimum Certification/Licensure: Certification by Illinois Department on Aging(IDOA)
