POSITION SUMMARY
The Admission Therapist plays a vital role as the primary facilitator for all admissions being presented to Wellbridge. The Admission Therapist is responsible for gathering and assessing patient clinical information for the purpose of making appropriate admission decisions. The Admission Therapist routinely interfaces with external referral sources, patients and internal staff to facilitate the successful admission of the patient into Wellbridge. The ability to build excellent relationships with all patients and their families is key to this position.
RESPONSIBILITIES
- Complete full phone screenings and integrated assessments to obtain information concerning substance use, mental health, medical history and all other pertinent information.
- Provide initial diagnosis
- Communicate clinical information with the treatment team and management to ensure quality patient care
- Completion of all OASAS required admission documentation including LOCADTR and consent forms
- Works closely with the Director of Admissions in providing oversight of bed utilization, reporting daily census and ensuring capacity levels are maintained.
- Acts as a primary liaison between the program and community/providers by making it a priority to respond to phone calls promptly and inquiries regarding requests for planned and impromptu assessment.
- Inform clients, referral sources and friends/family, with consent when necessary, about program-specifics and responds to questions in a professional manner with emphasis on easing anxiety, building trust and confidence and offering clarity.
- Effectively communicates with clinical/medical team members and in a timely manner regarding incoming admissions and other client-specific information that the team must be made aware of.
- Performs related duties, as required.
REQUIRED EXPERIENCE AND QUALIFICATIONS
- LMSW, LCSW, MSW, or CASAC, required
- 2-5 years' experience with working in the Substance Abuse, either in a clinical or admissions-related role,
- Motivational interviewing experience a plus
- Ability to conduct assessment utilizing best practices of counseling and crisis intervention.
- Ability to establish and maintain cooperative professional relationships.
- Ability to gather, analyze and evaluate information pertinent to the screening and admission process for clients with co-occurring substance dependence and mental health issues.
- Knowledge of principles and techniques of evidence-based counseling.
- Knowledge of community resources; federal and state laws and regulations regarding client confidentiality
- Take initiative and work independently
- Working knowledge of Electronic Medical Records
- Ability to work independently as well as in a team environment
- Ability to express ideas clearly, concisely, convincingly, and effectively when addressing an audience
- Knowledge of Substance Abuse and Mental Health
- Strong interpersonal skills needed to facilitate encounters with patients, staff, nurses, physicians, etc.
- Proficiency in Microsoft software, (Excel, Access), required.
